Is the ride over?

I have been on triple blockade for the past 7 years. Dr. Lam put me on Jevtana plus triple blockade to buy me more time on triple blockade and my PSA went from .4 to 1.4 in 2 months. It seems strange to me that it would go that high that quickly with the protocol I'm on. Has anyone else seen or herd of this.

Could you tell us the exact drugs that comprise your triple blockade? Generally it will be either a GnRH agonist or antagonist, plus an anti-androgen, and an 5α-reductase inhibitor. But there are a number of different drugs in each of the three categories.

Not that I am likely to have a clue about your question but it never hurts to ask the obvious things while we wait for the smart guys to notice your thread.

So you started the Jevtana something like three months ago and your PSA seems to have immediately started to go up, is that right? Have you had a chance to talk to Dr. Lam about the rise in PSA? If so, what did he say?

Sometimes after a man has been on ADT3 for a while (the triple blockade) the Casodex starts to cause trouble. My oversimplified understanding of how this happens is that Casodex works by attaching to the cancer cells' androgen receptor sites so testosterone can't attach and activate them (because the Casodex is in the way.) This prevents the receptor sites from being activated and presents the cell from dividing. But... After a man has been on ADT for a while his cancer cells will sometimes evolve so that they activate when Casodex attaches. This makes Casodex work like testosterone for those evolved cells and the cancer starts to grow again. When that happens the process can sometimes be stopped (or even reversed for a while) by discontinuing the Casodex.

It is possible that something like that may be going on with you. That would make the fact that your PSA started to rise after you started the Jevtana just a coincidence.

As I've just demonstrated once again I am not any kind of an expert. This is just something to talk to your doctor about.
