High Grade Stats for Stage 3 & 4

Hi I have created this thread to get an understanding of how treatment plan differs for high grade Gleason’s 8-10 and what are the outcomes.

This thread can help many of us make decisions or to get an idea whats next.

I’ve read numerous posts with similar diagnosis and pathology however treatment plan/outcome were completely different i.e. adjuvant/salvage/RT/HT.

I understand most of the info is there in your signatures, however this post will work as filter and Wikipedia for all the high grade warriors.

Please if possible in reply section can you please post the following details –

PCa Details

• Gleason Score/PSA before treatment ;
• Pathology Outcome ; pt or tc
• Positive Margin ; (Y/N)
• Nodes Involvement ; (Y/N)
• Bladder Involvement ; (Y/N)
• Mets ; (Y/N)
• Others ; (Y/N)

Follow-up & Treatment

• Adjuvant/Salvage with time from surgery/RT(wks/months) type i.e HT/RT or both.
• First PSA rise or if PSA is stable please mention with duration
• Change of treatment plan with reason
• next treatment plan i.e xtandi or chemeo.
• Current condition
• Future plan

Interesting idea for a post. I like aggregated information, and it could be helpful. Everyone's decision choice involves a lot of variables, including imponderables like our anticipated tolerance of different side-effect profiles.

Your survey seems to be designed for the surgery/salvage RT scenario. We don't all follow that path.

• Pathology Outcome ; cT3an0m0 (though my uro onc was ambivalent about maybe cT2 vs cT3 from MRI)
• Positive Margin ; (Y/N) NA, radiation primary
• Nodes Involvement ; (Y/N) None visible on CT or MRI
• Bladder Involvement ; (Y/N) None visible on CT or MRI
• Mets ; (Y/N) None visible on CT or MRI
• Others ; (Y/N)

Follow-up & Treatment

• Adjuvant/Salvage with time from surgery/RT(wks/months) type i.e HT/RT or both.; None yet
• First PSA rise or if PSA is stable please mention with duration; PSA up as T recovers from neoadjuvant HT, now at 0.5, won't be considered recurrent until if and when it hits 2.0.
• Change of treatment plan with reason; none yet
• next treatment plan i.e xtandi or chemeo. ; if recurrent, likely to resume immediate and continuous HT
• Current condition; feel great, curative treatment plan complete, monitoring for eventual recurrence
• Future plan; PSA checks quarterly, probable HT if PSA shows biochemical recurrence

Any one else pls with high gleason??

I was Dx G9. My treatment plan consists of IMRT 39 Factions 78 Gy + 2 yrs HT. Currently awaiting my first post IMRT PSA test and third Lupron shot. Was hoping for RP at the start ,then was told I was not a good candidate for surgery. Then scheduled "Triple play HT+ IMRT+BRACHY, but my urinary difficulties prompted my medical team to drop the BRACHY. Ended with HT+IMRT as my primary treatment. Now waiting to see how it's working.

Nice idea for info summaries. I'm a G7with advance PC so maybe I will start one too on different thread.

Posters can make it easier for themselves if they copy your template to a blank word doc, then paste the template to that doc, fill out their info, and then copy and paste it back to their post.
