SRT and Medicare

I'm just very curious, and couldn't find the answer using Dr Google.

Obviously SRT is covered by Medicare. But is there a cutoff? For example, would SRT be covered when PSA reaches .1, but not at say, .03, or .05, which some studies show is more optimal with poor pathology?

I don't know the answer with Medicare but I went for SRT when my PSA reached .03 and insurance covered the treatment. There was no debate or hassle with my insurance company.
