Interesting...my uro surgeon said it would improve over time. I only have intermittent leaking at orgasm, sometimes more than others, and sometimes none at all.

Hmmm....I have cut down on alcohol consumption, but probably drink more than I should. This should give me reason to cut down even more.

Ok, I'll weigh in too. I had the urine thing happen 2, maybe 3 times in the early months. It did get better and is basically a non factor now. Orgasms are near what they were prior to surgery too, so all is good there. BTW, sex with a full or nearly full bladder was always a deal killer before surgery for me, so I expected nothing different after RP. I usually gotta hit the head prior. Also, Tall Allen is right as he usually is, the Cowper's gland still works fine here.

I also shoot pee after surgery. At first I was embarrassed by this, but I'm not any longer. My problem is that the pee starts to leak out and as soon as that happens I start to loose my erection. Then my penis slips out of my wife. If I desire an orgasm, then I must ********* my soft penis and it shoots out pee onto my stomach. I wish that it would not leak out, but rather just shoot out inside of her. She doesn't care what comes out of my penis- semen or pee. She says it feels good having some fluid inside of her.

I have experience the urinary discharge during orgasm once and that was probably because I hadn't emptied the bladder beforehand.

For me, the difference between orgasms before and after my RALP is like having enjoyed fine craft beers for years and finding out that the only option forward are Lite beers. I question whether it's even worth it.

Aside from suggesting you pee before sex I got nothing for your leaking during sex.

I think you've mentioned using ED pills in the past otherwise I'd suggest those. I think the most fruitful approach would be to treat your problem as erectile dysfunction and try to find a way to fix that. When you were 18 it would have taken a lot more than a little squirt now and then to cramp your style.

The reason that men leak when aroused after prostate surgery has to do with how their plumbing used to work during sex. An intact man has two urinary sphincters, one on the neck of his bladder behind his prostate and another one in front of his prostate. As he becomes aroused the primary sphincter (the one on his bladder neck) tightens to keep urine from leaking out and the secondary sphincter (the one in front) loosens to facilitate ejaculation. When the magic moment arrives semen is dumped into the part of the urethra between the two sphincters and the muscular contraction force it out like toothpaste squirting out of the tube. After surgery the primary sphincter is gone but the other sphincter still relaxes when the man gets aroused. ... which makes him leak.

I've picked up a few tips in avoiding climacturia from patients:

• limit fluid intake in the hours before sex - especially coffee, alcohol, and citrus and other fruit juices.
• empty out the bladder fully before sex -try sitting and changing position to get maximum emptying
• use a constriction band during sex
• if you can, avoid taking diuretics and food and herbs that encourage diuresis (e.g., celery, asparagus, pineapple, bananas, pickles, garlic, etc.)
• avoid changing position during sex - most tell me that staying flat on your back is safest.
• ED meds may make it worse, but trimix is OK

Some guys and their partners have learned to like it. They say it adds to the ejaculatory experience. There are rubber sheets available for the purpose.
