Your 10 year probability of survival is 99% regardless of what or any treatment you choose. The 1% mortality rate was confirmed by Klotz and all were variants that looked like a G6, but were in fact a rare variant that caused death within 5 years regardless of treatment.
If it is a true G6, which should be confirmed by another PC pathologist like Epstien at Hopkins, and given your low psa, AS should be your first choice. The principal behind AS is that the majority of G6s don't have to be treated, ever. The only way to know which G6s progress vs those that don't progress is to watch them over time. Treat if progression is shown or continue to watch if it remains stable.
Bahn is one of the best in discovering PC by CDU guided biopsy. He only targets suspicious areas, that's why only 1 core was positive. If the cancer is too small to see by a very experienced CDU doctor then it is too small to hurt you. Once you get a pathology confirmation on your biopsy sample you can reasonably be assured that the staging is correct and you have a very low risk cancer.
Dr Bahn found my cancer after 12 biopsies over 10 years and one MRI failed to find it. I would put a lot of faith in his ability to DX pc and get it right.
