another new guy

Been lurking awhile as a guest and it seems there is quite a wealth of information and knowledge here. figure I might as well stick a round a while (hopefully years) if you guy's/gal's don't mind. So I filled out a signature and decided to post it. Hopefully it makes some sense.

At present just waiting for next PSA (July) to see where we go from here. Not real optimistic I'm going to end up in the cure category based on past history of staging upgrades and the persistence of PSA after salvage radiation. Researching local Medical Oncologists anticipating the need to go systemic vs. curative.

0.10 and going down...Nothing wrong with that...But yes, your urologists job is done, find a good MO to take the lead from here on. See if your primary care physician can refer you to one...

Just to add to the chorus about your psa so far after SRT, mine continued down for over a year before going undetectable and has stayed that way for over 6 years. Loads of factors involved but so far so good in my opinion in your case.

Looks like good numbers to me. Still in the curative zone.

Waiting for the PSA tests can play tricks with the mind/emotions.

Go live some life and do something fun and enjoyable until July.
Then, do share your PSA result so I can say, "See everything is fine."

Perhaps you should get a second opinion from another urologist or oncologist. As I understand it, radiation does not result in an immediate, dramatic decline in psa as you would observe from RP. Instead it goes down slowly. Perhaps you can get a better explanation from your current doctor as to why he is so concerned.

Welcome to the forum nodog. I'll join the others in noting that your PSA trend actually looks good so far. Of course, I'll leave it to those here who have more experience with SRT.

I'm trying to make some sense of your sig. It looks like you went from a clinical T2c following the biopsy to the pT3b pathology post RP... not uncommon, same happened to me. But other than SVI I do not see any mention of a positive margin which might indicate metastasis outside the prostate bed. Also I see a LN+ followed by -LN... So was there lymphatic involvement or not?

Beyond that, I'd say your stats look pretty good, other than of course the SVI, for which I can only say, "welcome to the club". There is a "T3" club here on HW... just search for "horses have left the barn" to have your name added to the long list of members!

While you're fixing your sig, your biopsy stage should actually be "cT2c" for 'clinical'... it isn't actually "pT" until the prostate has been dissected and examined for 'pathology', hence the "pT".

Just splitting hairs... at least, if I understand correctly how it works. I'm still kind of new to this myself.
