Hello all, I was just diagnosed 2 weeks ago with pc. I am 55 yoa, my psa have been slowly raising the last year up to 4.66. My biopsy results are below confirming pc. Spent over an hour with my Urologist going over the test results and my options. He made it clear it is my choice on which treatment I chose. He is recommending I have RALP done due to my age and the one GS of 4+3. I have scheduled appointment with both the surgeon and the radiation oncologist. I have found lots of information on the internet, but yet at the same time I feel as if I have found no information. It is nice to have found this forum where I can read personal stories and not just facts.
Right base of prostate, needle core biopsy:
- Adenocarcinoma, acinar type.
Gleason score: 6 (3 + 3), (grade group 1)
Number of cores involved: 1 of 2
Total length of tumor: 1 mm
Percentage of tumor: 5% of specimen volume
>50% tumor in any core: No
Perineural/lymphatic invasion: Absent
B) Right mid prostate, needle core biopsy:
- Adenocarcinoma, acinar type.
Gleason score: 6 (3 + 3), (grade group 1)
Number of cores involved: 2 of 2
Total length of tumor: 9 mm
Percentage of tumor: 45% of specimen volume
>50% tumor in any core: Yes
(This is the optimal block for additional testing)
Perineural/lymphatic invasion: Absent
C) Right apex of prostate, needle core biopsy:
- Adenocarcinoma, acinar type.
Gleason score: 6 (3 + 3), (grade group 1)
Number of cores involved: 2 of 2
Total length of tumor: 6 mm
Percentage of tumor: 25% of specimen volume
>50% tumor in any core: No
Perineural/lymphatic invasion: Absent
D) Left base of prostate, needle core biopsy:
- Adenocarcinoma, acinar type.
Gleason score: 7 (4 + 3), (grade group 3)
Number of cores involved: 1 of 2
Total length of tumor: <1 mm
Percentage of tumor: <5% of specimen volume
>50% tumor in any core: No
Perineural/lymphatic invasion: Absent
E) Left mid prostate, needle core biopsy:
- Benign prostate tissue.
F) Left apex of prostate, needle core biopsy:
- Benign prostate tissue.

Get a second opinion on the slides. Have them sent to Epstein at Johns Hopkins.

Sorry you have to be here. I am a recent member and have learnt a lot from HW veterans.

As you can see from my signature, I chose RALP. My close friend similar age who got diagnosed around same time chose "triple play" (HT+EBRT+Brachy) at a very renowned univ hospital.

I'm happy with my choice of surgery as its behind me now and I focus on recovery and glad that path report overruled MRI of EPE and previous biopsy gleason score ( Path downgraded gleason from 4+3 to 3+4). I'm relatively very new to HW but surgery is a pain for one week and today is the 10th day and I almost feel normal except for minor issues with Incontinence.

I would suggest to get a Multi parameter MRI to see if PC is contained and it also helps surgeon plan your surgery based on MRI results if you are taking the surgery route. I changed my surgeon last minute as I felt the previous surgeon only had 200 surgeries under his belt and my new surgeon had over 2000.

I'm pretty new also, my husband's details are in my signature. We are very pleased so far with his decision and he has minimal SE's. I would strongly recommend you review the first post on this forum, there is so much detail in there and it will help you get a better handle on things. The options available are also listed there in great detail with pros and cons. There are so many veterans here who will help and guide you on this journey.

Welcome to our unfortunately not so exclusive club. Your path report is fairly good and like other have said, lots of options for treatment. The huge dilemma we have all faced is surgery or radiation treatment (or AS for the lucky guys) and if the choice is radiation, which kind.

I have no regrets with my surgery choice, based on biopsy of the prostrate and nodes, that moved me into another risk category and was a trigger for additional treatment.
