The Gleason 9 Crew -- Part 2, continued.). The primary type 5 grade is the critical factor in your Gleason score. That factor is a significant risk factor, and not very common. It means this is something you'll want to verify, and throw the kitchen sink at it.
Have you considered a second opinion on your slides yet? Your urologist can take care of that for you. Mine were reviewed by Epstein's operation at Johns Hopkins. It cost me $250 4 years ago, but the confidence it gave me was worth every penny. Do you have any details, like how much of how many cores was positive? That can help assess the situation too.
Radiation is a good option for the high risk cases, with various methods and combinations. The combo of brachytherapy (either seeds or HDR) with external beam, adding hormone therapy, has very good results. The risk of side effects is increased due to the brachytherapy, so it's something each person has to decide. Can you live with the side effects if they happen to you? It's so hard to say before it happens. Anyway, just something to consider. The oncological control seems to be the best with that option. Your dominant type 5 cells warrant very aggressive treatment.
Have they discussed your staging yet? Do you know how well contained it is, maybe by an MRI? It's important to know that when developing a treatment plan.
