Fuzzboxman.....I completed SBRT (Cyberknife) a couple weeks ago and prior to treatment, had an in depth conversation with my RO (Dr. Debra Freeman) about
the enema necessity as well as the low fiber/low residue diet. Dr. Freeman indicated that the digestive system is quick to respond to dietary changes and that adherence to the diet and the need for an enema prior to each treatment depends on the digestive efficiency and stability of one's individual system and the time of day that the treatment occurs. Since I had to drive from Orlando to Tampa for each treatment, I tried to schedule each session in early to mid-afternoon. That enabled me to get up in the morning and have my "usual morning routine" which is to drink two cups of coffee and immediately take a dump. Since my treatment would take place before any more "poo" was likely to move into the lower intestinal area, no need for an enema. The Drs. are only concerned about
keeping the lower intestinal area near the prostate clear of substance to avoid putting any pressure on the prostate area (which would cause the rectal wall to be closer to the prostate). As for the diet, I observed that for the last meal or two prior to each treatment. The radiotherapist who actually performs the SBRT process can see if you have excess movement occurring in the prostate area during treatment and if for instance you have gas, they can see it and have you get up and go to the bathroom to relieve the issue before continuing the treatment. I went through all five treatments without any issue from gastrointestinal issues at all. The radiotherapist said that all the treatments went extremely well and that my preparation worked quite well.
I suppose it's possible that since everyone's system is different and some people are more prone to intestinal turmoil, the enema and diet could play a more significant role in how smoothly each treatment goes. My treatments lasted about
45 minutes each after being positioned on the table and I slept through almost all of it. Keep in mind that my treatment regimen was Cyberknife and the treatments last longer than the faster machines require so the prep can play a more important role.
Also, regarding the enema prep specifically, I had enemas before all scans, biopsy, fiducial marker placement, etc. I told Dr. Freeman that I was buying enemas by the case and was considering buying stock in the company since enemas were being required for seemingly every procedure along the way, even the consultations. We all had a good laugh......at my expense since I was the one having to perform that wonderful procedure repetitively...
Nothing like a good enema to start each day.......particularly since Drs. and hospitals frequently schedule scans and tests early in the morning. Getting up at 4am and immediately sticking a tube up one's rear is really not what I consider an ideal way to start one's day......
Good luck with your treatment. My experience with SBRT has been nothing short of incredible so far. It's been almost without side effects of any consequence so assuming things continue this well, the real issue will be how the efficacy of the treatment is for me.......my PSA will tell the tale.
