Prostatitis is hard to treat and often doesn't respond to antibiotics. UTI and retention could also bump up the PSA. Typically PSA has a serum half-life of around 3 days. That you had a drop after the first round of antibiotics is a good sign. All things PSA tend to happen slowly and often just one test isn't enough - you need to look at the trend. If the trend is downward, cool, don't worry. An upward trend is a trip to the doc for answers.
Guess how much PSA should drop is a difficult game - too many variables to accurately predict. Likely you should wait another 3 months and retest. If it's lower, then you know the prostatitis is clearing.
Biopsy? You'd be one of the very rare cases to have PC at your age. In general, you want to rule out all other possible rises in PSA before even thinking biopsy. Honestly, I'd be surprised to find out that you have PC, even with the high PSA.
Wait, retest, and then see would what I'd do.
What did your doctor say about this?
I know that the thought of having cancer is scary, but PC is a different game. Even if the worst is true, you have so many treatment options that likely it won't be more than a health care bump in the road. There is every possibility of a cure and beating that.
