Bone scan done today. It showed lesion at right illeosacral joint which is progressing.. No other lesions were found. Oncologist told that radiation will be helpful. Will spot radiation be helpful or should radiation in whole pelvic region be given? Also let me know about
radiation therapies like SBRT/ IMRT???
Diagnosed prostate cancer 2014
Bone scan clear
Initial PSA 8
Radical prostactomy august 2014
PSA after 3 months 5.0
Bone scan showed one met in rib and one at lower back.
Started hormone therapy zolodox and casodex january 2015
PSA jan 2016 0.4ng/ml
stopped casodex
Bone scan showed lesion at sacral ala and lower rib
PSA april 2016 4.5
started taxotere
PSA after 3rd cycle 1.5
PSA after 6th cycle 0.6
PSA after 21 days of 8th cycle 0.4
After one month PSA 2.8
Started Abritrone acetate
PSA after 3 months 5.9
Zolodox changed to Firmagon
PSA after 3 months 15
started Antiandrogen Cyprotene acetate
PSA after 15days 18
PSA after one month (07/09/2017) 27

helpful in what way? If there is pain in the joint, RT could help with that.
Whole pelvic radiation doesn't seem helpful to me.

No, patient is asymptomatic. Medical oncologist advised that by giving high dose spot radiations PSA will get lower.

The only reason to radiate a metastasis in his case is to get relief of pain or prevent a fracture. They can accomplish that with as little as one treatment. They have to treat his cancer, not his PSA.

I prefer spot radiation with SBRT. This way you apply a higher dose to the met and achieve better local control. Also the radiated area is smaller compared with IMRT and you could radiate again if a new met appears close to the existing one.

Thanks, George. Discussed with radiation oncologist, he said IMRT will be helpful. Does radiation completely kills the cancer cells? and does anyone having idea of side effects of IMRT??
