I did a lot of research last night and went through all my old papers and found 2 very interesting things. 1. My Humana HMO list Moffit as an in network provider! 2. I found my last BX report, it is 3 years old and my 1 core in LB is 3+4 so my Gleason score is 7 not 6 like I thought and the volume of 68cc was after 9 mo of AD so God only knows what it is now. I don't know why my URO wouldn't do another BX and he placed the gold marker in the one spot in LB using a 3 year old BX?????????? I think the OC was treating the whole prostate or he could have been off a hair and was hitting the sphincter muscle and urethra accounting for all the damage done, just guessing because that would account for not doing a new BX if he was going to treat the whole gland! So good news on the Insurance, bad on the G score and prostate volume. I am going to call Humana now and see what I can do. Humana was very good when my wife got sick and we didn't have to pay for much at all, probably less than $1,000 for the whole 2 years and everything the PCP and other Doctors requested. I found out that Humana offers a medigap if you wan to triple your premiums so I'll find out what the out of pocket expenses will be and go from there!

Thanks, BensonHill.
Philmire-
You are right that the whole gland was treated. The gold fiducial markers are put in place to help aim the beams to correct for organ motion. They based the radiation plan on a CT/MRI taken after the fiducials were placed, so the previous biopsy really doesn't enter into it. Your prostate size must have been a whopper before hormone therapy if it is still 65 cc now, and that may very well explain your elevated PSA back then. However, your GS 3+4 puts you into an "intermediate risk" category. So I hope that you find an experienced surgeon somewhere with whatever insurance you decide upon.
