Metastasis forecast

-Does anyone know if a test like the decipher test can help me in forecasting if/when, will I get metastatic pc please?
- my RO told me a year ago it takes 8 years but I forgot to ask him when you start counting. Is it from the time of surgery? Time of RT? Or When psa becomes detectable as in my case now, my psa is .2 after surgery and radiation failed ?

I heard Howard Scher isn't taking new patients, and I suspect that Philip Kantoff isn't either. Michael Morris would be a good choice.

AK - if I may say, you should find a medical oncologist who specializes in prostate cancer. You've already had RP, which essentially failed, and SRT five years later, presumably without concurrent HT. You are already at the point of BCR by all standards (0.2) and the only benefit to waiting any longer would be to accurately establish your doubling time. The time to act is now.

Roblee, I never had HT when I had the SRT. I truly believe I have BCR. But my question is when you say the time to act is now, you mean go on ADT now and not delay it? There are conflicting studies
That shows no much difference in survival or castration resistance if you do it early or later. some people think you should start ADT in these conditions(high psa, Mets or short DT). I might be wrong but I think I don’t have any, although my PSADT is questionable. I am very confused

My thinking is if I know I am heading towards ADT no matter what?and since my psa is at .2 now, what would be the harm of taking a mental break for 6 months. I will continue to watch ofcourse and get a psa every 3 months or even less. I am working on D & E now hopefully it might increase the PSADT. even if psa jumpbed to 1.0 within 6-7 months I am still in safe area to start ADT. I am trying to forget about this darn cancer even if it’s only 7 months. I haven’t slept for 10 days now since I got the news about psa .2

Can any image scans find Mets at .2 psa?
If it founds any Mets, can they radiate now?
The reason I am asking is that I do not want to miss a chance that they can help with something instead of me waiting 7 months and loosing this window without doing anything. But if they can see it and don’t do anything about and start ADT, then I would rather wait. Please advice. Thanks

It's doubtful, but possible. You can see the detection rates for all the best PET scans at various PSAs in the table of the following:

And whether met-directed therapy is of any value is unknown. But you have little to risk in pursuing it.

Interested in following this thread. My husband's PSA is at .4 and he's scheduled for AXUMIN PET at UTSW next week. His PSA doubled from .23 to .4 in three months and Urologist was ready to start Lupron the same day. My husband, resisted, wanted to think it through. The most recent PSA test was done at a different lab using a different ASSAY which may have affected the results. But, the lab report says the latest test is typically 20% lower than the other ASSAY. So that would make his PSA closer to .5 He's retesting PSA this week at the UTSW lab vs our local lab to confirm.

I've been searching for information on doubling times after Radiation Therapy. He was 5 years-post RT 1/29/18 at his last Urologist appointment. His lowest PSA was reached at .21 about three years after he finished RT and it's been consistent at .21 or .23 for past two years until 1/26/18.

His radiation treatment was the primary treatment. He had Lupron prior to starting the RT.
I will start a new thread at your suggestion. I don't have all the gleason info, etc but I can fill in what i know.

At first, we thought the Urologist was over-recting, but we also wondered if we were in denial after coasting along with level PSA of .21 or .23.

Husband is 64 and had a lot of medical complications November, December (diabetes diagnosis, venous inefficiency and gout). This led him to start focusing on weight loss and he started about 5 different meds, including Metformin. I've researched impact of his meds on PSA levels and haven't found anything that leads me to believe it led to his PSA bounce.

But, we are reluctant to dismiss the urologist's counsel at UTSW (Dallas). He literally said, "Every urologist in this office would start you on Lupron today or order a PET."

I don't understand why Lupron would be the doctor's first line of treatment if he doesn't even know if it's a recurrence.
