Zytiga must be taken "with meal"? Said his doctor

Hello all,

As some of you may remember, I was going to provide Zytiga for my father with mets. PCa and finally I managed to buy it for 3 months.
The doctor said he should take it "with" a low fat meal. We do not have access to him for 2 weeks because of Iranian new year celebration.
Does anyone knows "with" means between or after the meal? Or no difference?

It's the opposite - his dose should NOT be taken with meals. Here's what it says on the prescribing instructions:

• Take ZYTIGA on an empty stomach. Do not take ZYTIGA with food. Taking ZYTIGA with food may cause more of the medicine to be absorbed by the body than is needed and this may cause side effects.

• No food should be eaten 2 hours before and 1 hour after taking ZYTIGA.

The dose is 1000 mg, all at once.

However, if he does take it with a meal in order to make it last longer, it should be ahigh fat meal, and he should take half the dose (500 mg).

It must be taken with prednisone (5 mg taken twice a day)

He should, of course, continue taking whatever medicine he uses (or orchiectomy) to prevent any testosterone from forming.

Farz53-- do you know what the dosage of your fathers prescription is? Is he taking a generic form? A study was done that showed taking 1/4 of the dose of zytiga with a low fat meal had the same effect as full dose with no food.

I think that article got it wrong (which isn't surprising - the media are often inaccurate). The presentation at the ASCO meeting that they refer to is below. It says that men who took either 250 mg or 500 mg (they lumped them all together as "low dose") with a HIGH FAT meal had equivalent effectiveness to men who took 1000 mg. I would be wary of going as low as 250 mg.

Whatever dose of Zytiga you decide on, be sure to obtain and take the prednisone also...
