JP drain levels post prostate surgery

Age: 67; PSA 8.4; Gleason 3+4; 64% positive cores on biopsy; perineural invasion; tumor stage T1c
Procedure: open prostatectomy 6.11.18; Home on 6.13 with JP drain and catheter - both still in.
JP output on 6.11 - 315 ml; 6.12 - 325 ml; 6.13 - 285 ml; 6.14 650 ml; 6.15 580 ml. Dr. recommended taking suction off 2 hours / on 2 hours, but after 2 hours of having it off, when I put suction back in system, fluid leaked out of JP insertion site, draining the dressing, my T-shirt and shirt. Because of these levels, dr. ordered creatinine level lab test of JP drainage. Awaiting results. No fever, pain improving, bowels need 'convincing'. Overall doing well and catheter is scheduled to come out on the 25th. Anyone else having this issue of JP output increasing instead of decreasing? Thanks, I'll keep you posted.

Sometimes this forum is slow in response, experienced posters will be by soon I’m sure. I didn’t have surgery is your catheter draining ok with normal amount? Waiting for results, three evil words for sure.

Never had this, altho8gh it took a little longer than usual for them to feel comfortable removing the drain, but only 2 days, not 5. Wish I could give you some advice, you just have to trust the doc. Hopefully there will be someone here who sees this and responds.

This sounds unpleasant, so I feel for you. I had surgery and a drain, but the drain came out pretty quickly, before I left the hospital. Was there something unusual about your surgery that required this kind of drain? I also had robotic rather than open surgery, so that may be a difference.

I recall having the drain in for the entire time I had the catheter - more than one week, and the fluid never seemed to let up. I also recall the amount went up some days, down other days. I still had a lot of fluid even until they removed it. I had thought it had to go to zero, but that was not true. Don't be discouraged - the amount of fluid might depend upon your hydration status and other factors.

Thanks for the feedback. It’s much appreciated. Urine output seems less than past days but is good color. Just noted leaking around tube even with suction on! Still no fever or significant pain. Waiting on lab report still. Some numbness noted in left lower pelvis.? I’ll keep you posted.

Same here, the resident pulled my JP drain out just before release from the hospital (I was in the hospital for one day post surgery) and that was a big ouchie!

Cath came out after about 8 or 9 days.

I remember draining quite a bit of fluid. I did have some leaking problems but my main problem was tissue blocking the tubing. My wife did not have the stomach to help me clear out the tube.

I leaked a lot where the drains entered my body. My wife taped pads over the incisions to try and keep me from dripping (changed 2X a day)

The drains and staples came out after 7 days and catheter came out on the 15th day

Like a few others, I had my drain and catheter removed after 7 days. We kept the suction on the drain the whole time and logged the output. It was still producing a bit on day 7 but not enough to keep it in. It did leak a bit but the dressing took care of of it. Once those two were out I was free to travel home. I went to Cleveland from Montana. The only thing I had afterwards were daily shots of heparin in the gut for a little over 3 weeks.

It’s been 17 days. JP drain still in. Output is now between 200 ml to 250 ml. So it’s starting to come down. I took catheter out yesterday. That’s going about as I expected. But the JP drain is way out of the expected norm. I believe I have a lymphocele. The surgeon (surgery was done 90 minutes from home) is on vacation so I’m dealing with his PA. I’ve also been emailing the Urologist here in my hometown. They both want me to sit tight until the surgeon is back and get instructions from him. I’m having some pelvic discomfort but Advil or Tyelonal help. Any thoughts? Hope all of you are doing well.

If the PA (not just you) suspects lymphocele, I'd think it would be worth having an ultrasound before the surgeon gets back and finding out for sure. Most lymphoceles resolve on their own after a few weeks, and don't require draining or other invasive procedures unless they get infected. I developed a lymphocele, it made me miserable for about a month, could hardly walk, but then it went away.
