Finally had a look at my pathology report. Trying to understand it

I finally looked at my pathology report. I had gotten married and had been out of town for the last month.

I had robotic surgery at UCI by Dr. Thomas Ahlering on April 18th. I was his 1741 patient.

I am now 6 weeks out and have no incontinence. I also am able to get an errection although I have to admit an orgasm is nothing like it was before the surgery.

Here are the highights:

A. Anterior prostate fat, excision: Benign fibroadipose tissue.

B. Adenocarcinoma, ductal-type, Gleason 4+4=8 (grade group 4), left mid medial, 0.7cm
-Additional foci of adenocarcinoma

Acinar-type Gleason 3+4 (15%) =7 (grade group 2), multifocal, bilateral, largest 1.1 cm
-Organ confined

Extraprostatic Extension - Not Identified
Seminal Invasion - Not Identified
Lymphovascular invasion - Not identified

Margins (specimen):

Apical / Distal Focally
Positive in anterior, <2mm
(anterior prostate fat -negative)

Bladder neck/Proximal: Negative
Prostate surface: Negative
Perineural Invasion: Not Identified
Vas Deferens: Negative for tumor

Pathological staging: pT2.

What the surgeon told me was that they found foci cancer cells on the surface of the prostate but the surrounding tissue was negative for cancer cells. Don't know if that is going to be a problem or not.

Any comments?

That anterior foci margin was on the prostate. The prostate was removed. The anterior fat removed around the prostate was negative so I am hoping for the best.

What Halbert said. Not horrible, except the G8.

You'll need the first PSA test to know better how things stand. For now, you're looking good.

Your report says "Adenocarcinoma, ductal-type" (if I read it correctly). Its unusual, nearly everyone with prostate cancer has Acinar type adenocarcinoma. Ductal is an aggressive variant of prostate cancer (you were G8!). Good that you were treated early when it was still contained.

Since you knew going in that there was G4+4 involved, I would say that this is pretty good, and could easily have been much worse. The absence of EPE and SVI is very important. Now it is on to the waiting game. Good luck!

What fiddlecanoe said. Seeing as you’re continent, I think drinks are in order. I’ll buy you one on Saipan tomorrow night, but if you’re not there I’ll have to drink it myself.
