The phrase has no single fixed definition. An example definition would be a combination of a low number of positive cores (three or fewer positive cores, or 25% or fewer cores positive), no positive core having more than 50% involvement, and no more than 4mm length of positive involvement in any one core. An upper limit on the amount of Gleason 4 found in any one core might also be added in.
