That ain't right. I had my prostate removed nearly 10-years ago. I take daily Cialis, and have (and enjoy!) daily erections.

The pump doesn't do anything that your (or somebody's) hand can't do to get the blood flow going which is important to prevent atrophy. The pills do, on the other hand, so it's not a question of either/or...it's typically pills plus pump (or hand, or something).

That's not at all uncommon with Viagra. Other people have unfavorable reactions to Cialis (although not typically both, since they are different.) Reactions also cannot be predicted based on any individual's body chemistry...so with PDE5 inhibitors (as it is with just about any class of drugs for which there are multiple formulations), it's a trial-and-error effort to tune-in to the drug that works best for you as an individual. Cialis is a completely different drug formulation (tadalafil) than Viagra (sildenafil), and both are completely different than Levitra (vardenafil).

And that ain't quite right. A pump will pull enough blood into the penis so that with a ring to hold it there, a very hard erection can be produced, even if a hand and/or any amount of pills result in nothing what so ever. With the later not being uncommon.

However, some find the pump so un-natural and weird feeling that they don't find it worth the effort for use during sex. Especially if used for more than a few minutes, since the trapped venous blood- with no fresh arterial blood coming in to replenish, starts getting cold. And the ring can become quite uncomfortable, depending on how tight you need it. But, it can convert an otherwise dead organ into something quite capable of penetration, even if nothing else has worked.

For those who get zero response to pills(and that is more than a few), injections will also very often produce a usable tool. But they can also be quite uncomfortable for some of us, might only work for some when standing up, have a chance of sending you to the ER with an erection that won't go away, and plenty of people who develop Peyronie's (curved penis) had been injecting first. The authorities say injecting does not cause Peyronie's Disease(PD), but it is for sure that plenty of people who inject have reported developing PD. But for some folks, injections work wonderfully.

And then of course there is the old Penile Implant. Rock hard after a few pumps, every time. But you might not be able to orgasm or have a pleasurable orgasm, might be a lot smaller than you used to be(due to the surgery), and of course it is expensive(unless insurance covers it) and has the risk of surgery. But, a lot of folks really love them.

Even though a few folks report reasonable satisfaction(go to Franktalk for lots of info), I don't like pumps personally. But I have to disagree with your statement "The pump is really no more effective....... to the more organic alternatives".

What are you calling the more organic alternatives? A hand? A pill? I can assure you that there are guys here that an entire pack of Viagra and hours of stimulation from a beauty will produce nothing but flaccidity, but who coud, with a pump, produce a true piece of wood. Would it be as enjoyable as a natural erection from the old days? NO! Would it be as pleasant as a Viagra and stimulation induced erection- or even stimulation without Viagra- for those for whom such can actually attain such a result? NO!

But there are more than a few of us here that no amount of any of that is going to do anything after that doc cut or damaged our nerves. It is not going to happen even after years of healing. And for many of those- who have enough desire for sex to go through the hassle- a pump can give them a maximally hard unit that can sure enough penetrate. I know- I have done it many times- and even used it successfully for sexual intercourse during my first year post op. Believe me, there was nothing organic that was going to accomplish that.

Can a penile injection accomplish such? For some of us, yes. But that is hardly organic(heck, neither are pills) and has a bunch of negatives of it's own. But for many men, no amount of pills and/or stimulation is going to accomplish what a pump/ring, penile injection or penile prosthesis can. If the nerves are cut, the former is not in the same ball park as the latter.

The pump is a "durable medical device", my insurance paid for mine - the name brand. A lot more than $75 too.

When they take all of the nerves, you no longer get nocturnal erections which are critical to keeping the spongiform erectile bodies flexible enough to fill with blood and give you an erection. If you use the pump for 15 - 20 minutes every day to replace those nocturnal erections, you can prevent the buildup of scar tissue that will prevent you getting erections.

I would have to use the pump and a ring to be able to stick myself with the needle, which I have thus far been unable to do. Still do the daily pump sessions to keep the physical parts I have left functional.

The daily Cialis is supposed to increase blood flow in the area after surgery, which is why I took it for a couple of years. Never did anything else for me though.

Since Eligard killed one of my testicles and messed up the other, my T is hovering around 200....so is my libido.

There are about a million variables that determine which solutions will work for a man who had developed ED as a result of prostate surgery. Here are a few of them:

1) Damage to the erectile nerves. Depending on the location and the extent of the cancer and the skill of the surgeon the erectile nerves may or may not be damaged during surgery and that damage may or may not heal to some extent during the two to four years after surgery. If the erectile nerves still work but not as well as they used to then pills might help. If the erectile nerves are shot then pills won't help.

2) Low libido and loss of confidence. Hormone therapy is the worst for this but most men are impotent for three to six months after surgery and that is hard on anyone's confidence. After a few failures men become afraid to try and that nervousness makes the problem worse.

3) Venous leak. The veins in the penis are designed to be squashed flat by the pressure of an erection, reducing the blood flow out of the penis and helping maintain an erection. A constricting ring can help somewhat with this and those rings are often used with a pump. A venous leak will also interfere with pills and with Trimix injections. Men with venous leaks find they need larger doses of Trimix (because the drugs wash away as the blood leaks out.)

4) Penile atrophy. Men with normal erectile function get several erections every night. This keeps the tissues stretched out and oxygenated. Without at least several erections a week the tissue starts to deteriorate making the problem even worse. A pump can help with this. It does a pretty good job of keeping things stretched out but is only so-so for oxygenation. The pump pulls more blood into the penis but that blood is a mix of arterial blood (with is well oxygenated) and venous blood which has much less oxygen.

5) Lack of a partner. about the time men tend to seek treatment for prostate cancer, their wives are going through menopause and losing interest.

I had RARP with one nerve bundle spared 10 months ago, but no natural erections yet, although I do get some minor lengthening and thickening, same as I have since 2 weeks post-surgery. Nothing has changed or improved since then, was on low dose Cialis for 8 months, then high dose, and also Viagra, but no response at all to pills. Also no response to stimulation of any kind, other than the minor response.

I am impatient, (my wife also, though she pretends to be patient), not interested in injections, and already thinking about implant, but docs said I should wait at least a year post surgery to see if natural erections come back. Some docs say 18 months, as I had no erection issues prior to surgery.

They have recommended I use a pump a few times a week to help prevent atrophy, and the $30 kind works perfectly well for that. Some of the more expensive ones are better designed to facilitate transferring the rings onto the base of the penis, but I have so much venous leakage the rings don't help much.

I would suggest you try a cheap ring or two first, to see if that helps hold whatever erection you can get. If the rings work, then maybe a more expensive pump will be better if you're having sex, as it may be easier and quicker to get out of the pump and into your partner. If you're just preventing atrophy, an inexpensive pump is fine. Just make sure it has a pressure gauge and release valve.

With a skilled implant surgeon, the spongenaous material in your penis(inside your tunica) is just moved over to make room for the implant tube. IF,IF, your nerves come back you may still have natural erection as well as the inflatable implant.. Some guys with implants do have morning wood. My URO told me he removed all my nerves so I had an implant 9 months after the RP. I also had penile/ incontinence PT right after the RP. Was instructed to use cheap VAC every day to keep blood flow to my penis. Implants done properly do not cause shrinkage. The amount of time from RP without blood flow causes shrinkage. Before RP, 6 inch, after implant 6 inch / 5 inch flacid. Boston Scientific 700 lgx will expand to the entire capacity of your natural tunica.

One question- did you see gradual improvement, or just one day it worked again?

I had RP 7 months ago. I have been taking 20 mg of Viagra daily and the results continue to improve. I also use a pump daily to help the blood flow. Whether the pump is actually doing me any good or not I don’t know but things keep getting better so I’m sticking with this plan.
