I can't really answer as I have been in the group for which RP was the end of sexual pleasure since RP 5 years ago. But some guys do much better, and may you be in that group! And if you are already having erections with or without pills, it sounds like you will be. Good luck to you!

The achiness in the perineum is a common short-term complaint post-RP and could be around for months, but the good news is that it is not an indication that the necessary 4-week healing process is incomplete.

Consider Sunday morning (or Saturday morning) rather than Saturday night for your first attempt(s). At the end of the day, our aging bodies are more-and-more fatigued (true for both sexes) and our bodies revive and regenerate with a good night's rest. If you want to stack the odds in your favor for the best possible outcome, consider (especially initially) attempting morning sex. (This is particularly true if you, like me and my wife, like to enjoy a cocktail or two in the evenings; the alcohol also can detract from performance.) What you may find--and what I found--is that mornings work out great, and now we set our morning alarm 30 minutes early to take advantage of nature's regeneration cycle.

You may also want to keep in mind these sildenafil parameters--it typically takes about 30 mins to be effective, maximum effectiveness is at about 1-hour, and the half-life is only 4-hours, but the therapeutic window may last for 12 hours...so knowing that there is no "prescribed" time of day to take it, consider this input. The much longer half-life is one of the things I particularly like about tadalafil...it's really made for ongoing, continuous consumption rather than the "on-demand" approach for sildenafil or vardenafil. This may be a longer-term medication adjustment for you to consider...but for now, you just want to demonstrate success.

All of the different PDE5's are different...so, again, keeping the longer term in mind, if you experience any unpleasant side effects, I would encourage to jump to one of the other types. They are completely different meds which work in completely different ways biologically (to achieve the same final result), so there is no reason whatsoever to believe that side effects from one will also mean side effects from either of the others...just move on to the next one. It's unfortunately a trial-and-error process.

I suppose the strict instructions are helpful when first getting going (and it sounds like your doc's regimen takes into consideration many of the relevant points I mentioned). This strict adherence won't be needed in the long term...but first things first, I would urge you to follow the instructions you've been provided closely for the best possible outcomes. (Many men get far fewer instruction and are merely handed a prescription and told, "good luck!")

Depov - I’m on a similar protocol only the pump was every other day for 15 minutes. I met with the ED therapist after a month and he was fine now with at least 3 times per week but for me to do it now 1-2 hours after the MWF pills which I now take in the mornings.

The pump gets pretty boring after a while and you have to plan time for it since there is prep and cleanup. My wife only watched me once for a few minutes and that was that. I told her she could borrow it like a breast pump and she was not amused. I told her it might work better if she worked the pump and that was the last I saw her at pump time. Good luck, hope you blow through this part too.

Paul, good point--but the other recommendation, building on what you're saying, is take it easy. Low impact. This is not a time for experimenting with exotic positions. Yes, plenty of foreplay--many of us find that it's different--for me, it takes a LOT of direct stimulation to get things going today, so that's a reality.

Depov, take it easy, after 4 weeks you won't injure anything if you're careful and not rambunctious. Good luck.

OK. I would have to say that things did not work out as I hoped. However, it is early.

Saturday was nice foreplay, then a moment of penetration and then... Discouraging. I guess you have to start somewhere. It seemed like there was more leakage than ever before just from the erection. I think that kind of freaked me out and ruined the mood for both of us. Looking for improvement next time. I know I have to be patient.

This was also the day that I was to start the pump. Not much success either. It just seemed to stretch things sideways- looked like a mangled blob. Is this normal? I read that when you have Peyronies you can't use the restriction device or you can cause permanent damage. Makes me want to send it back for a refund (too late?).

Did manage a dry orgasm without the erection. Things are different for sure.

Depov - You are doing fine, I am not so lucky quite yet. I have the "leakage" issue with the "dry" orgasm and my wife got to see it firsthand (pun intended). It has gotten a little better but my body wants to let go like old times except the wrong stuff comes out. I can stop it somewhat by concentrating on the kegel muscle but the minute I let up, away we go with a spurt or two. Like you, it seems that certain stimulation can involuntarily relax the muscle and can cause a spurt even though I have no other continence issues. You can look up the subject on the site and find lots of people have had these issues, it was the first thing I did and was glad is was not just me.

The pump should work although it is not that much fun and takes time. I assume you have instructions or have been instructed. Make sure everything is lubed up (use water soluble Aquagel or eq.) so that it does not stick to the side. Take it slow with a couple of pumps, pause, repeat, etc then hold. It is a little clumsy at first but you get the hang of it.

My urologist group has a class at 6:00pm once a month where you get/buy the pump (some people's insurance covers it, Medicare does not apparently) and a person gives instructions on how to use it. This particular model (Osbon Esteem Manual) has you use gel around the end to make the seal and to lube the contact areas. It comes with a case, rings, inserts, etc. You can spend even more $'s and get an electric verson.

Floor sounds like a lot of work pretty soon to me. I waited 2 months before doing any real work then started to ramp it up, same with working out. Now I have tons of snow to deal which has been no problem now but could have wrecked me a while back.

I am not really sure where a random spurt comes from, seems like a muscle relaxes for a just moment to me. Have not had it happen with all the stress events that should cause it like sneezing and lifting so beats me.

After an orgasm (not during) there is some sperm leaking out. How can this be if there is no prostate or seminal gland? Definitely not urine.

Saw the doc today and he said that with the amount of scarring he saw on the nerves (3 biopsies), he didn't think that there would be even a partial erection for at least 6 months. He was surprised to hear otherwise. So I am ahead of schedule. Good news because I thought that I was not doing well in that area.
