pT3b with PSA < .010 for 9+ months after RALP. First annual lost the coveted < and was reported at .018 and immediately began monthly testing using uPSA. Had discussed with URO my desire to implement SRT at .030 which he approved of. Met with MDA associated RO and went over my data he also supported my decision to begin RT. Asked RO what I would need to do and was told when PSA rose and I desired to begin to call his office to schedule simulation and be scheduled. He said he approved of my decision after our meeting entirely. PSA went from .018 to .021 to .028 them .031. At that time I implemented action even though my .031 was followed by a last data point of .025 as that rounded to .030. Both URO and RO states ADT was entirely my decision and I opted for 6 months ADT beginning early last December. Handled Firmagon well, just three days after shots of inflammation and soreness which was handled with cold packs. Read a lot of comments about
shifting to Lupron with negative SE so again I made decision to inject only Firmagon. Ended with last shot in early May. Began RT in January and had last fraction on March 14th. Had no SEs from IMRT whatsoever. On Jun 17th had first 3 month PSA after completion of RT which came back as < .010. Had them run a T check for baseline after ADT. Last ADT shot was on May 3rd and on Jun 17 my T was measured at 16 ng/dl. After the 6 months of ADT and 1 1/2 months after termination I tend to view a 16 as a good sign. Other than occasional mild hot flash I have no SE presently from either RT or ADT. Rather long comment for me but just wanted to pass on info. Maybe it will be of interest to someone in their decision process. Blessings to all.

You did how it’s supposed to be done!
I hope you have killed those nasty cells for good.
Just curious, how young are you.

Wishing you continued good results, but I'm curious, given your adverse pathology (T3b/SVI, tert 5) why you waited sixteen months to initiate ADT and SRT? ... Personally I had to jump thru lots of hoops to get ART underway within 12 months of RP.

ddyss - In three months I will be 70 years young.
RobLee - Debated ART but in discussion and agreement with URO decided to wait. I wanted at least one year for healing and continence. Which I achieved. Additionally, as long as I was seeing PSA < .010 I was satisfied with watching. During this timeframe is when I discussed with URO my firm intent to implement SRT immediately upon identification of increasing PSA levels. As with all things associated with this beast only time will tell. I made my decisions and will press forward. You take care.
