Second Opinion from Dr. Epstein at Johns Hopkins

Working on getting a second opinion from Dr. Johnathan Epstein at JH to confirm Gleason 6. Biopsy was at Mayo in Phoenix. Mayo is telling me JH must request to biopsy slides, and JH is telling me they do not request them - that Mayo must send them.

Catch22. Surprised that these two hospitals have this level of confusion.

For those that have used JH and got a 2nd opinion from Epstein - what was the process you dealt with?

Waiting on a call back from the Mayo Path. Dept. for further info, but would like input from the community first.

At my first visit with the Uro following my biopsy, I asked the Uro to send my biopsy material/slides to the Dr. Epstein lab at JH for a second opinion review. He then made the request through the hospital Pathology department and they sent it. In this case, it was a doctor ordered test. The Uro also had the tissue samples sent to Oncotype for testing. That test requires actual biopsy tissue samples, and is not a review of the original pathology slides. In their online site, JH indicates how to have either the slides sent or how to have tissue samples sent. The latter requires the samples be sent in a paraffin block.

I was also told that I could request the samples/slides be sent by filling out a "Release of Information" form from the hospital and request Pathology forward the material to JH.

Asking the Uro to have them sent was the easier of the two methods.

I just did this last week. I used the forms and sample letter that is in the link that pratoman posted.
I emailed those to the pathology department at UCLA. They had their own release forms that I needed to complete.
Now I just wait for them to send them to JHU.
Easy peasy ...

Thanks All. Took a few days of back and forth - was obviously talking to the wrong people. I have it all sorted out now - slides will go out from Mayo Phx to JH tomorrow.

I also asked two weeks ago for the Onco DX test - was told the Uro Doc or his staff would send off for it. Will need to go back tomorrow and check on the progress of that.

Glad you got it all sorted out. I had mine sent from Columbia Presbyterian to Hopkins with no problem, just a little paperwork. Everyone was very helpful at both institutions. Then I had the results of both sent to Sloan. Again, everyone was very helpful along the way.

No. Almost though. My urologist was at Columbia and he did the biopsy. I then sent the slides to Dr. Epstein (Hopkins) for a second look.

In the meantime, I was not comfortable with the radiologist at Columbia so I sourced Dr. Zelefsky at MSK. At first MSK wanted the slides themselves for their pathologist to review but when Dr Zelefsky saw the two doctors who had already done them he accepted the results without MSK repeating.
