PET Scan shows cancer in nodes after 5 years of RP

My husband went through a PET scan as PSA was rising, it showed 4 spots, 3 in pelvic and one in chest area (Medistinal node station 4R). Although the doctor could have operated on Pelvic, he said that the chest one was inoperable. My fear is that the prognosis is not good. My husband is not 55 and the cancer seems aggressive. He thinks hormones are the way to go, this time with Xytandi.

Princess Margaret is the best hospital in Toronto but we would like to see if there are any surgical or radiation options globally that we can try but there is too much information on the internet to sift through. Any advice that you offer on this forum would be a great help.

Yup, Xtandi would be the next step. Spot radiation is sometimes possible on mets. See if you can talk to an RO to evaluate that.

Toronto, I believe Dr Eugene Kwon at Mayo Clinic in Rochester, might be worth talking to. He treats oligometastatic Prostate Cancer with Radiation. It is somewhat controversial, as to whether it extends survival. But might be worth a visit if you as you say , are interested in exploring it globally. Not sure, maybe he would do a phone consult? Here’s a video from 2014 where he discusses the topic. Be aware things might have changed over the last 5 years.

They are Medical Oncologists who only treat prostate cancer. Very well known and well regarded, and many here, including me, have consulted with them, and many if those (not me) have been treated by them. They do phone consults, when I talked to them it was $750 (US), but that was 5 years ago.

In the video Dr. Kwon does not show the situation of the patients he presented about a year after his treatments. From what I learned in the meantime, usually new mets appear after treating the visible mets in about a year or two. So these patients were not cured as the video suggests. The disease progression has been delayed. This is a benefit, but not what the video suggests.

One objective of this video is to market the Cholin PET/CT Dr. Kwon has helped to develop.

Thank you for all your responses. I have heard that there are is great advancement in technology in German university hospitals which is a highly sought after destination for medical tourism. If you have come across anything new in Europe, we would be willing to try it.
