RADIUM 223. Rising PSA. Any known compatible blocker?

I have just had 4th treatment of Radium 223. My PSA is going thru the roof, now 300. Should I have a blocker with the Radium? If so which ones are compatible? I have had zoladex and more recently aberatone.

Enzalutimide (Xtandi) would be next, then an estrogen type treatment such as DES, estradiol by patch, gel. Then maybe dexamethasone. Ask your oncologist about trials you may qualify for.

Gary, can't really comment on your question, and sorry you have to be here, but I'm really glad you found us.

Knowledgeable people will probably be along shortly, there are many here with a lot of different stories.

If you could post some more info about your diagnosis and treatment history, it will help folks give you better info.

And you can't find a better place to ask your questions, vent, complain, whatever you want to do. We're here to support each other.

Hoping you find something to control your situation. That sounds like a lot to deal with.

Welcome. It is important to stay on a GnRH agonist or antagonist like Zoladex or Lupron even though your PSA is rising. Even though it seems to be failing, it is still keeping your testosterone down. If Zytiga has stopped working, try Xtandi. Provenge is a good idea too. One man on this board was able to get Cometriq off-label through his insurance, even though it's not yet FDA approved for PC. Also, I agree that clinical trials can get you some medicines not otherwise available.

Some Oncologists are giving Xofigo (Radium 223) as a combined therapy with either Zytiga or Xtandi. Lupron or Zolodex is given as well.
My husband is on the above treatment with Xtandi but I don't have much information as he has only been on it for 2 weeks. Time will tell.

My Father starts his first Xofigo treatment this Friday. I read it is sometimes used with Xtandi. My father was on Xtandi and did not work for him, is that why his oncologist did not add it to his treatment? Since i did not know about it I did not ask his Dr. On 11/25 PSA was 186.70
12/8 PSA 315.52
Will the Xofigo bring his PSA down?
Also, can anyone tell me how they felt after taking Xofigo, did they feel any immediate improvement. I know it affects their blood count.
Any info would really help!

I’m just starting on Radium 223. I hope your questions got answered. Xtandi didn’t work for me either. I’m taking Radium223 in conjunction with Docetaxil (Chemo). How did your father fare in his treatment?

Yes I do. I’m hoping to hear a success story, as I look into Xofigo with doxetaxil, post- XTANDI failure...

Also looking for verification of my understanding: am I right that Xofigo is not intended to drop ones PSA?

Ok. Regarding PSA, I have seen articles in UROToday that indicate it can reduce tumor load and reduce bone pain, but doesn't always have an effect on PSA. That is in line with your understanding. I assume its effectiveness would come from pain relief and decreased tumor size from scans.

I also suggest you check in on the other forum where Tall Allen now resides. It is oriented toward advanced cases and may have some relevance for you and he clearly had information five years ago.

The general idea is that Xofigo will be absorbed by bone metastases because Ra (radium) is chemically related to Ca (calcium). Or, stated somewhat differently, the body treats radium as calcium and deposits it in bones.
Clinical trials have shown that Xofigo is no wonder drug, but I do hope that combining it with chemo will help you.

A more effective radio-pharmaceutical is Lu-177, not approved in this country but available in Germany for several years.. $7500 cash per treatment plus travel expenses..You must test PSMA positive for this treatment to work.. The scan that determines your PSMA status is hard to get and expensive..80% test positive, 20% do not..It's not a cure but it might give you another year or two...

You should be cautious about saying a treatment may give a person 1-2 years. We have had many guys on this site with a worse prognosis live much longer than 1-2 years. I know you did not mean coming across as not having empathy, but your post comes across that way.

These radio-pharmaceutical treatments are usually reserved for stage 4 terminal PC patients who have failed both ADT and chemo. The mortality rate for this group is high and few live more than 5 years when they reach this point.. There are exceptions to every rule of course and a few of us achieve long-term remissions and live to die of something else..
