First, if all cores showed cancer, it probably means there is a lot of cancer in the prostate itself. It doesn’t necessarily mean that it’s escaped the prostate. You should request a copy of the full pathology report and keep it for future consultations.
Second with your relatively low PSA it’s highly unlikely that it’s advanced and your bone scan will likely show nothing alarming
Third, the most important thing you can do right now is request that the urologist send your biopsy slides for a second opinion to Dr Jonathan EPSTEIN at Johns Hopkins. Prostate slides are particularly hard to read, somewhat subjective, and often, local pathologists read many different types of slides. Dr EPSTEIN is a world renowned pathologist who reads only prostate slides and is recognized as the gold standard.
IF HE UPGRADES YOUR GLEASON SCORE/GRADE, OR DOWNGRADES IT, IT COULD INFORM AMD CHANGE YOUR BEST TREATMENT OPTIONS.
your Dr will know how to send it he likely gets this request often.
Fourth, you should consult with experts at a major cancer center if possible, or at least at a major teaching hospital.
Assuming the G7(3+4) is correct, you have many options, and good chance for a cure. There are different types of radiation, and there is surgery. Speak to an expert in each of these disciplines, and don’t bother with opinions from surgeons about radiation, or vice versa.
I could go on, but fir the sake of brevity, I will end by saying G7(3+4), which under the new system is Grade Group 2, is considered LOW intermediate risk and is highly treatable, so you have a long future ahead of you, PC won’t get you.
