Gleason 10 update and any suggestions

Hi I am back on the forum after sometime to provide update And I would also need your comments regarding current issues I am facing thanks.

For past few(4-5)weeks I am spotting little drops of blood at the end of urination. Ive no other issues. On my recent visit last week to my oncologist he did regular 6 monthly checkup includes all basic tests(psa, urine etc)and 6 monthly Lupron injection. All test came fine however the only thing he pointed was thickness of bladder is classed mild. Not sure this could be the cause however he said he will monitor for 6-8 weeks and if required will do cystoscopy as most likely this is caused by radiation cystitis.
He has prescribed Diclofenac Potassium and Desloratadine in the mean time as no infection in test no antibacterial tablets.

Anyone has any experience around blood in urine mine is regular 4-5 weeks with gap of day or two sometimes only 1 drip noticeable though.

History:


Age 63 when Diagnosed, current Age-67

Nov 2015 – Urination issue prescribed urimax for next few months

Jan2016 – Postarate size 28gm

Jul2016 – After taking urimax urination issue still persist

Jul-2016 – PSA 6.9 doc advised to wait for 8 weeks and retest

Aug-2016 PSA 12.44

--MRI DCE No enhancement ‘heterogeneous signal in peripheral zone’

Sep2016 – trus biopsy 12 cores 30% area.Gleason score 5+4.

Sep-2016 Bone scan – As per the radiologist scan is negative

Sep-2016 FDG PET CT Scan negative.

Da-vinci date to remove prostrate– 04-Nov-2016

Catheter out – 14-nov-2016

Final Pathology – pt3b n1 mx. Gleason 5+5, 21 lymph nodes removed and tested – 1 found infected in left side.(not the result I was hoping for)

First PSA 6 weeks after surgery - < .008

Feb-2017 .01

Mar 17 – .03

Carried out PSMA scan and found prostrate bed affected with Ca.

May-2017 Lupron 6 months injection with Calutide 50mg daily started.

24-07-2017 Radiation 7 weeks IMRT(vmat-79gy with Calutide 50mg).

PSA since May-2017 < .008.(Most recent last week Sep-2020)

Nov 2019 MRI of back with contrast came negative as I had back pains for few weeks.

In agreement that a telephone call to a trusted urologist would be wise.

I am sure you are experiencing some worry and unrest, and left wondering.

You have everything to gain by making that telephone call tomorrow.

It can take a while, to get an appointment --- and it would be wise to describe your specific situation, in case they want to see you "sooner, rather than later" ...

For what it's worth, I wouldn't be comfortable with the "let's wait and see" for the 6 to 8 weeks your oncologist suggested.

Brotherly advice ---- I feel it's time to have a consultation with an experienced urologist --- you have experienced the passing of blood for four or five weeks already --- and the problem persists.

Wanting the best for you, Jack1954 --- be proactive, and get an appointment booked --- peace of mind, taking that step --- and then seeking your doctor's expertise.

I had a similar problem and it turned out to be a bladder stone found by a 'scope exam...Stone removed (using the 'scope) and problem went away...
