Hi,

For those that may have seen my earlier post. My boyfriend, 9 years after RP, had his PSA go up. It slowly climbed from year 7 to year 9. We pulled the trigger on SRT (without ADT) when he hit .13. He did not want ADT and neither urologist or radiation oncologist felt it was mandatory. Finished radiation in mid - Aug. Had a 1 month PSA drawn at his urologist. PSA has come down from .13 to .04. Seems like it is responding. Since surgery is so easy... either you are detectable or not, I am assuming this is a good sign. Not sure how quickly it is supposed to fall or how long it will take till you get to nadir. So I've read, PSA after SRT has a half-life of approximately 1.5 months. Seems like we are tracking to that. Any thoughts?

Thats good news. Take it and run with it. Its a sign that your SRT hit the right spot.

Yes, that PSA drop sounds like some serious good news. Already in your favor is how long it took for the PSA to become pesky once again. Based on the idea that whatever might have been left behind by surgery, if it was aggressive, likely would have become detectable long ago. Some studies have shown that men who have recurrence = or > 3 years after surgery have much improved odds of long term survival. So, both of those things are now in your favor. Good news indeed!

Teddydog, you boyfriend situation is exactly like mine. My PSA started rising after 9 years. In 04/29/2021 it was 0.12. I also pulled the trigger on RT (no ADT) 0n June 26th and finished on August 23rd. I had a PSA test on 9/22 and it came back the same at 0.12. I'll discuss the result with my Urologist next week. I was hoping for a lower PSA like what happened to your boyfriend. You should celebrate the news.

Ed C (Old67), I am sorry to hear that you did not see a drop as you had hoped. It's hard for the surgery folks to understand how the PSA is supposed to respond with radiation. I've done some research that medium time to nadir is ~4.6 months. Some go a lot slower. I hope in your case it is a slower drop... but it drops. It didn't go up, so that is a good thing. It could have crept up in the 2 months between PSA and radiation start. The next PSA should tell the tale. Unfortunately... like it always is with this disease... you have to sit, wait and worry for a while. God bless for a positive response.

Teddydog, thanks for your response. My oncologist told me that the first test one month after completion may not show a decrease. May be the PSA staying the same for 5 months is a little bit of good news. I'll be testing again in 3 months and that will be the real test on whether the RT hit the target.
