I'm curious when they cut the fellows out, do they take some of the scrotum skin?

Since you asked how I got there. Shrinkage due in large part to surgery and non-use. Well, when the big guy shrinks, he hides in the scrote skin. Thus making leakage a bit messy, soaking the scrote.
As I'm about to the point where I may as well stop ADT since my PSA has been slowing rising, I thought about castration, which since we had been talking about shrinkage, I thought maybe the castration might take extra skin and therefore reduce the soaking of the nut sack.

Don't ask me to explain, because lately I feel like I've been in a late 1970s mind.

It's completely optional. The most common simple scrotal orchiectomy just makes slits in the scrotum to access the testicles. They're excised, tidied up, and the incisions simply closed. Nothing else removed.

There are surgical options, and maybe your surgeon would be willing at some additional cost, to remove the scrotum entirely. It's a much bigger surgery, since the scrotum walls are not simple skin. There are multiple layers of tissue there, and getting it to a good outcome that heals well, and even with a decent "aesthetic" outcome, takes skills more common to a plastic surgeon. Urologists aren't usually "elegant" about the job.

Commonly it is a two-step process. Simple orchiectomy, and a later scrotectomy by a plastic surgeon. It's two surgeries, two anesthesia events, and so on. Probably more costly that way. Some do both at once, but I wouldn't probably have a typical urologist do that unless I could verify with some former patients to make sure they were pleased with the outcome.

And in pursuing this, you may have to accept the idea these types of surgeries are often considered "transgender" treatments. While that is not our goal of course, it's just the marketing under which these things are being presented. It's something we just have to deal with.

Alephnull, Redwing’s reference is excellent. I am in Atlanta and I have heard of this doctor. I remember an interview with him a few years ago regarding genital reconstruction work for a lot of different situations. I can’t vouch for his quality, but I can certainly ask my urologist about him.

A consult with him, or at least a contact with him to get a suggestion of a practitioner in your area could be valuable. As I understand he has become significant in the speciality of genital modification/repair/reconstruction for both cis-gender and trans-gender people. On his website he shows both orchiectemy and penis lengthening, for example. Thus dealing with the relation between penis and scrotum, He may have procedures that would help you. Additionally, he is probably experienced and effective in working with third party payors for reimbursement. Some plastic surgery is medically necessary, usual and customary, which is the requirement for reimbursement. You could probably get a Tele-medicine meeting with him and he could observe your condition and provide some guidance.

Also in your area, look for urologic surgeons that do penile implant work. They may have procedures that can help you. More off the wall, but perhaps effective, is to contact LGBT advocacy organizations in your area. They will know of medical practitioners that are good (and bad) that they refer transitioning people to. There is significant physical transitioning surgery happening in many of our major cities and the relevent practitioners doing the surgery would be to ones to search for, in my opinion.
