Ray, Modern radiation can be safe, convenient and very effective. The risks of secondary cancers are very low. Most estimates suggest it’s hard to measure and probably less than 1%. Certainly a good talking point with the experts at MD Anderson.

Although surgery is likely to cure both your BPH and PCa in one procedure; it does come with significant risks of side effects. You may wish to consider treating both conditions separately. The advance’s in treating BPH are amazing and many are compatible with RT. HoLEP, green-light, Aquablation, and others are all worth investigating. We all learn from each other so keep us posted on your visit with MD Anderson. Terry

As ASA said, those comments about secondary cancers are likely outdated. Don't get radiation advice from a surgeon and don't get surgical advice from a radiation oncologist. You seem to be doing all the right things including extra view of your slides and contacting multiple practitioners. A quick update. I just got my second opinion report from JH, and looks like my Right Apex (C) got upgraded from 4+3=7 to 4+4 =8. A little bummer as i thought it might get downgraded. Anyways, it is time to move on for treatment. Will be asking questions related to RP and SBRT/IBRT in separate posts. Thanks.

Beware of urologists badmouthing radiation treatment. Be especially wary when they provide inaccurate information about radiation. Surgery may well be the best treatment option for you, but I'm glad you will explore all options. Tell the radiologist what the urologist said about secondary cancers. Ask them for journal articles that examine this topic.

Ray, I am with you on your Gleason 8 score. With a previous TURP the surgery path was a little hit and miss. I went with IMRT, again because of the TURP procedure. You may have more options, but ADT will probably be in the picture whatever path you choose. Try to relax and listen to what the vets here have to say. Your Uro sounds like he is not up to speed on all that has occurred over the last couple of years. Get more info from a good RO and see if you can at least get started on ADT to slow everything down. Think positive and ask all the questions you may have. It sure helped me to decide which way to go. My best to you.

Ray, Gleason 8 can never be said to be finally defeated. But it can be pushed back, made undetectable for good blocks of time, all the while with great QOL. Many levels of treatment possible. I have been at it for 12+ years. My best thought is to find an MO who will help you take the long view. It is a marathon.
