54 years old. Made a post that my PSA has always been 2.0 ish. Never higher. Last year I had pelvic pain/scrotal pain. When for a gazillion ultrasounds. Nothing came up bad and my prostate was a good size.

Pain in pelvic area started again a month ago. got a psa two weeks ago. 6.7. Doc tells me not to worry, puts me on antibiotics, thinks its inflammation. BUT:

1. there is no bacteria in my urine
2. my white blood cells dont indicate any bodily infection

So what can this be? Taking the antibiotics for 2 weeks, then waiting a week, taking a new PSA and seeing a urologist.

Thoughts?

Could be an urinary tract infection. Could be prostatitis. Both very common at your age. See what the antibiotics do. Calm down and relax.

Your symptoms sound like prostatitis. Most guys who come to the forum with those symptoms are worried that it might be caused by cancer. The thing is, prostate cancer seldom has any distinctive symptoms but prostatitis has all the symptoms you describe. The word "prosatitis" just means an inflammation of the prostate -- it can be caused by a number of things including, but not limited to, bacterial infections. Sometimes it is caused by an infection from which you have mostly recovered but a little bit lingers -- not enough to show up in your urine but enough to make your prostate cranky. That's why urologists often use a "treat to diagnose" approach. If you treat it with antibiotics and it gets better it was probably bacteria.
Chronic prostatitis that is not caused by an infection is called "nonbacterial prostatitis". You can Google for it if you want but don't expect what you find to be particularly helpful. It can be caused by a number of things that are difficult to diagnose and hard to treat. The good news is that it probably won't kill you or be particularly disabling. The bad news is that if the antibiotics don't clear it up you'll probably be putting up with it off and on for a while.
In the meanwhile it's a good idea to talk to a urologist. Prostatitis can make your PSA zig-zag up and down and there are better blood tests than just PSA for cancer screening.

Remember, however the "gazillion ultrasounds" and diagnostics you had a year ago. Take some comfort in that.

Talk to your urologist, describe your symptoms, and led your doctor lead your forward.

Likely, a round of antibiotics --- such as "CIPRO" will be prescribed.

It's easier said than done, but don't cross the bridge until you get to the river. One step at a time.

Let us know what your doctor says. They are there to help you.

Urologists often get booked up --- but they often offer appointments on a "cancellation basis" - in the meanwhile. That can lead to you getting in sooner, oftentimes.

thanks for this. the doctor has me on doxycycline for 14 days. Hopefully that will be long enough. he has also written me a referral to a urologist, so here we go!

I too think its an infection. What blew me away was the 6.7 PSA. Were it not for that, I wouldnt have began to worry. btw there was no bacteria in my urine, so a urinary tract infection is not likely.
