My husband called John Hopkins and found that they had already completed the 2nd opinion and sent results to our provider, but they were kind enough to email us the results below. I do have a couple of questions afterward down below if anyone is willing to help.
A. Prostate tissue with small focus of atypical glands, suspicious for low-grade adenocarcinoma. High-grade prostatic intraepithelial neoplasia cannot be excluded with certainty.
B. Benign prostatic tissue
C. Small focus of prostatic adenocarcinoma, Gleason score 3+3=6 (Grade Group 1) involving less than 5% of (1) core
D. Prostatic adenocarcinoma, Gleason score 3+4=7 (Grade Group 2) involving (3) cores (80%, 50%, 90%), less than or equal to 5% Gleason pattern 4
E. Prostatic adenocarcinoma, Gleason score 3+4=7 (Grade Group 2) involving (3) cores (70%, 60%, 70%), 20% Gleason pattern 4
F. Prostatic adenocarcinoma, Gleason score 3+3=6 (Grade Group 1) involving (3) cores (50%, 50%, 70%)
Perineural invasion identified
The pattern 4 in this case lacks large cribriform morphology
So it seems that there are 10/12 positive cores (my husband thought they took 17 cores, but either they only sent some of the cores or some of the cores were not usable and that's why there are less? I'm only guessing here)
So based on this, do you think he would still be considered favorable intermediate? PSA between 5-8 (first was 7.something and a month later it was 5.something), DRE detected node to left side, prostate 32 (I don't remember what the volume is measured in offhand).
The percentage of the cores seem high to me - how does this affect prognosis if it all?
We meet with his urologist on 2/1. We had requested a referral to RO (large HMO) by email, but the response was that we could request that referral at our meeting on the 1st (which was annoying). We do know that our HMO does SBRT and HDR Brachytherapy, both of which we are interested in and hoping to learn more about
. With these results, would you go with 1 treatment or do a combined treatment of some sort?
