That is bad news for sure.
The docs should be aggressively gathering information. Biopsy. Various scans. They are trying to assess a few different things
1) The aggressiveness of his cancer (gleason score/gleason grade group)
2) The extent of the spread. They will count the number of metastases they find and note the
location(s) (if any.)
As was mentioned by Mumbo PSMA Pet is state of the art for PCa.
This then sets the conditions for initial treatment. There are many combinations of treatment depending on the results of the tests. Generally speaking you want
1) lower gleason scores (3+3, 3+4, 4+3) vs (4+4, 4+5, 5+4, 5+5) See

You should also ask about
genetic testing. There are two kinds: Germline and somatic. Germline requires you to spit in a tube. Somatic testing requires some tissue from a biopsy or surgery. Both of these tests might find genetic signatures that can lead to treatment using immunotherapy drugs, with sometimes *spectacular* results if you have the right kind of defects. The defects are rare but when they are present some drugs can completely wipe PCa out. So push on the genetic testing.
There is an ongoing research study that offers a free germline test.
