Having the prostate removed back in 2013 and radiation in 2015 (thanks to a small positive margin) my PSA stayed undetectable (<0.02 Quest Labs Post Prostatectomy test) for 6 years. Then at the end of 2021, I lost the less than sign (0.02) until this month. I feared a slow increase in the PSA value, but I was pleasantly surprised by this month's PSA. I regained my less-than-sign of <0.02. Hopefully, this wasn't a fluke.

In Jan 24 I will reach my RALP 8th anniversary. I have my PSA tested twice a year; once by the VA and once through medicare at QUEST. The VA uses the ADVIA Centaur assay. Invariably, year after year, it comes back as low detectable .02. However, my QUEST is always < .02. In recent past years, I had it tested by Labcorp and it always came back at either <.014 or <.006. Why does the VA test come back low detectable? Who knows? Greater sensitivity? Since it remains constant at .02 year after year, I can only assume it's measuring benign prostate tissue left behind after surgery. My PCP seems to agree. The ultrasenstive PSA testing at such low levels is not exact science.

You would think that all of the tests would be reading the same values, but they don't. I guess there aren't any "standards" on PSA testing. I guess the bottom line is to use one lab that you are happy with and stick with it. And use those values to see if the PSA is sneaking upward.

Since I have both VA and Medicare coverage, I have the luxury of getting two PSA tests using different assays at my annuals. I like it that way. If both rise simultaneously it's time for 6 or 3 mo. tests. So far, so good. .02 for the last 6 consecutive years on one assay is noize. MD's agree.
