You said "...not even a hint of radiotracer uptake anywhere. " I believe there is a population of men (about
10% ?) who do not produce any
PSMA. That might explain one of the mysteries. However, your docs would know this, because most organs produce some PSMA (e.g. the lacrimal glands especially) against which the radiologist must normally identify malignant tissue and mets that "light up" more. In other words, perhaps "no hint of radiotracer uptake" meant "no hint of
abnormal radiotracer uptake."
The other mystery is that your MRI identified no lesions even with your PSA level. Perhaps a
benign piece of the prostate itself or
PSA-producing tissue was left behind during the RP and this has subsequently grown while remaining benign (?). (My uro brought this up again at my last visit as a possible explanation for my slowly rising but still very small level of PSA.)
