I should probably wait until the coffee kicks in but I don't necessarily think you're castrate resistant. It appears that your PSA started increasing not long after you stopped Orgovyx. Remember that this drug clears the system much faster than Lupron type drugs.

To me your PSA trend just means it may be time to go back on ADT. I will come back after my morning walk to see if this still makes sense to me.

Good to hear from you. It's nice when old forum friends drop by from time to time, even when it's because they feel the need to talk about what the Beast may be up to.

But as I think you'll agree, our crowd here is big enough, and experienced enough, so that somebody will have some good advice to offer. That's so often the case, since collectively we've seen a lot.

Have you been on xtandi at all? I’ve been on this as a monotherapy.

Hi Mel, PSA rise upon stopping the Ogo should by itself not indicate resistance. Been my pattern for near a decade. If you want to do a PSMA scan for possible mets, recommend Gallium as opposed to F-18 as the imaging agent. Gallium is the more sensitive of the agents, particularly with PSA levels below 5 or so. As for XTANDI, my MO at MSK is very high on the results of the EMBARK trial, results released last September, and easily searched. At the moment my treatment is pretty much whole cloth from that trial. Wish you well… Bill

Good to see you posting. In some ways I think Xtandi is shaping up to be one of the best 2nd gen anti-androgens, especially after the results of the EMBARK trial were published. My MO tells me Xtandi doesn't have the best side-effects profile though, but I don't know.

I sometimes consider going on Xtandi mono-therapy with the idea of maybe reducing side-effects. I don't think my MO would be on board with it, but even if he was I might be too anxious with it. You've been doing it for 2 years, so your MO is ahead of the FDA. He and you must be doing many things right AEB your history.
