Greetings, everyone. I'll add my 2 cents and it is probably worth about half of that. Everyone has to make their own informed decision and then go with it. That is the issue - getting to an informed decision.
In my case, I saw a urologist that had treated me a year earlier for a kidney stone. He was neither a surgeon nor a radiologist. He did my biopsies and gave me the news that 2 were positive and 2 were marginal. When he did that he said there were several options and the 10 year prognosis was very similar for all the options. However, at 10 years all of the options had issues except for surgery. If it was contained and the surgery got it all, most of us just keep on getting zeros. Thus, he considers the age of the patient in what he recommends. If the patient is around 70 then he explains the options and tries to help the patient make an informed decision. At my age of 55 he indicated he strongly recommended surgery. He doesn't do surgery or radiation so didn't have a "dog in the hunt" - he would be recommending me to someone else either way.
I chose surgery because based on my research, my urologist was correct. If I had anything other than surgery I would always wonder what was going to happen in 10 years. Maybe nothing but there would also be a good chance that I would have to do something else at about that point.
For me, I am convinced I made the right decision. My father was about 75 when he was diagnosed and he had radiation. He is doing fine at 81. His twin brother, my uncle, also had PC at about age 70 and he had surgery. He is now in his final days after his metasticized to his bones. Go figure. As with all options, you have to pay attention to the percentages. Just because 90% plus are cured with whichever option, you have to remember the other 10% and the fact that those are real people with real families who love them and want them to be there for years to come.
I believe the main thing is to make that informed decision and then have confidence that you are doing the right thing and then find the right doctor who is very experienced in whatever option you choose and then go for it and don't look back.
My two cents! David
