I've been using injections for 18 months now. There's learning curve and, yes it can have a little pain at first, until you master the technique. One thing for me was I had a bad habit of injecting too deeply, missing the cavity and getting into the tissue between the cavities, in the center of the penis. I solved that problem by developing my "plumping' routine, which consists of using the pump to give myself a little plumping or small erection then placing a penis ring on to hold the blood in. My condition is that the blood does not get restricted and when the pump is released, it flows out, taking the drug with it. Using the pump and ring and keeping the drug inside for better absorption results in a much better result, along with no pain whatsoever. The needle passes easily into the cavity (I can feel it 'drop' into it), and the injection is easy. There's lots of techniques and you can find yours. If you have been 3 years without an erection, then injections seem to be for you. I know it has done a world of good with my outlook on life and my ability to have erections again. We all have to face that initial fear, you're not alone there, but the results is worth every bit of it, count on it...
