Not approving of blanket biopsies for an elevated psa, but at least in my case, elevated psa, low free psa, "what the f?" comment during DRE, I felt like mine was justified. I'm almost glad I didn't know then what I know now. I just trusted my uro, and bang - 9 of 12 with 4+3 or 4+4.
And yes, every procedure has risks.
I am reminded of a book I read not long ago - author had mildly high PSA for years, and kept getting biopsies until one was finally positive. He was happy he "finally" had PCa. That qualifies as overtreatment in my mind.
