I haven't posted in a while about Dad - although I do check up on everyone regularly.

We had an appt 2 weeks ago. PSA was up from 0.6 to 2.7 (YIKES). Doesn't take a doctor to realize that's alot in just 3 months.

They ordered an updated Bone Scan and CT Scan. Onc. claims Cassodex is not in the treatment cards - she says there are "so much better drugs out there".

Test results are today - any advice for questions we should ask?

I've read alot about Provenge, and Zygita (although with Zygita it says they should try chemo first in order to be approved ?)

Scary time -- praying for good results from the Bone and CT Scans.

I'm in the same boat. After a year on Lupron my PSA rose from 0.6 to 3.79 in about five months. The next step may be Zytiga.

Zytiga is now approved for use before chemo. That approval came from the FDA just a few days ago so we may benefit from it.

My oncologist doesn't have much to say about Provenge. I know there are arguments both pro and con but I can't **** around here and Zytiga looks like a super-charged version of Lupron.

Please keep us posted about your choices and how it goes.

How about ARN-509? Still in clinical trial but lots of patients are having good results. Just a suggestion.

Sorry to hear you're in the same boat. Onc. said the bone scan showed a much "clearer" picture. The cancer is more solidified on these scans and mets is present in the spine, ribs (fractured due to mets/coughing) and one other bone I cannot remember. The original scans from 2010 showed these same spots just not as "lit up" I guess. The CT scan shows lymph node involvement around the ribs and pelvic area.

She recommends Zytiga - She said to take them a minimum of 1 hour before eating breakfast (4 pills of 250 mg each = 1000 mg dose). After 1 hr he can eat and then takes Prednisone (I think the Prednisone is given at dinner too - 5 mg at breakfeast and 5 mg at dinner). He will also have to start Xgeva (spelling).

She will see him two weeks after the start of treatment for lab works, will check the PSA every month. She has patients going 2 years on Zytiga with good results. She has others 3 months that blow through it. Hoping for the 2 plus years!!

We will read about ARN-509. I haven't read anything about that yet but am going to do my research. We definitly want back up plans for when the Zytiga stops working although it sounds like the plan would be Provenge, Chemotherapy, Clinical Trials.

Dr. Sawyers is co-inventor on ARN-509 and MDV3100, he stated in a blog thing I read a while back, that he thought ARN-509 is going to be even superior to MDV3100 for binding unto the AR receptor and MDV3100 binds better than anything prior invented, it appears, thus MDV3100 sometimes given the name 'super casodex' these are in the same family '----mides' (flutamide, nilutamide,bicalutamides, etc.)

Vary expensive unfortunately, generic casodex is really cheap, eulexin is reasonable, nilandron is more expensive stuff, but these newer ones are in another stratosphere and likely will work after you fail on casodex or other of the 'mides' family of drugs, hope insurances will pay for it all.
