So, I finally talked to the Dr. today about starting Trimix. He injected .3 cc(?) in the office two hours ago, and I'm still hard as a rock. I'm starting to get worried about it. Really don't want to go to the ER with this. I think I saw a post once about taking a sudafed to help. Am I correct about that? While my wife may love 2+ hour erections, it's a bit long for me. Maybe my next attempt should be just .2 cc's?

Yep you remember correctly. sudafed the one from behind the counter that you sign for is the one to use. You may want to call the Dr. He can give you an injection.

Hello cwren,

I cannot believe your doctor injected you with Trimix without keeping you at his office for observation or explicit instructions to follow if the hardness does not subside in a time-frame of less than four hours.

At two hours, I would be calling his office or making arrangements to be in an emergency room before the four (4) hour time limit.

I would also call ahead to the emergency room so they can contact your doctor before your arrival because they most likely will NOT KNOW how to treat you.

Sudafed and ice-packs, to the groin area, can sometimes help but I would not delay seeking medical help at four hours since this is your first injection.

I'll let you know. My wife just went to the pharmacy for Sudafed. The nearest ER is only about 20 mins away, but my Dr. is about 45 mins away.

At the 3 hour mark, I swallowed my pride and called a good friend who is the Dr. in charge at the ER nearest our house. I actually caught him at home and not on duty. He told me to give it another hour to see if it goes down. He said as long as it wasn't really painful or turning purple, not to worry yet. At the 3 1/2 hour mark, it finally went down. Whew! Guess next time I'm going to try just .2 cc's.

Glad it went down for you without a trip to the ER. I am two-time veteran of ER trips after trimix injections. At the time, it was not fun. But, I laugh about it now. Good luck with getting the dose dialed in.

I'm thinking I may be able to start a second career in ****. Maybe I could go by Dick Everhard, or Dick Everyready. :) Don't think my wife would go for it though. Lol!

cwren – glad you still have a sense of humor after your experience.  Sorry this happened to you.  This really makes me angry.  I’ve read about this happening too many times on HW -- an overdose of Trimix given by your doctor on your first shot.  And what makes it worse: he sent you home before it subsided.  And even worse still: he didn’t tell you what to do about it!  At least you are willing to try again.  I know of one guy on HW who had a bad experience like this and he’s afraid to try it again – even at a lower dose.

It’s hard to compare dose amounts since Trimix is custom blended as per each doctor’s instructions, therefore the strength can be different.  And each patient’s needs are different.

But to enable you to compare - my Trimix Rx is: Papaverine 30mg - Phentolamine 1mg – Prostaglandin 10mcg/ml.  My first injection given by my doctor was 0.05cc.  You started at 6 times that amount!!! After a month of slowing increasing the amount, as my doc instructed, I found the right amount for me to be 0.20cc (still less than what you started with).  And again you have to take into account the strength of the medicine.

IMO I think your next shot should be less than 0.20cc.   You know Trimix works for you; if you try a lesser amount and it doesn’t give you results, so what? – up the amount the next time by 0.01cc or 0.02cc until you find the right dose.  Why risk another incident? 

According to Dr. John Mulhall, a highly regarded ED expert, an erection, at or above penetration hardness, lasting 4 hours is considered a urologic emergency.  Erections lasting longer than 4 hours deprive the penis of oxygen which can kill erectile tissue much the same way that a heart attack kills heart muscle.  Men who’ve had several cases of priapism (4+ hour erection) are more likely to no longer respond to pills or injections.  He says 90 minutes should be the maximum.  If it lasts 2 hours you should take 4 Sudafed 30 mg tablets (not the extended release) in an effort to get it down under penetration hardness.  At 3 hours you should be in contact with your doctor and if it is still at or above penetration hardness at 4 hours – you should be IN the ER.

Yours sounds a lot like my Doc. Did the .30cc, beautiful 100% hard in his office .. sent me on my way with a "Hope you can get out of here with that" (And had to traverse a waiting area that had been empty when I went in and somehow had filled up with women 30 minutes later!). And almost a 4 hour big E. But I was already experienced with shots (plain PGE-1 ) for 6 months so I was not too worried. Did finally go down at around 4 hrs.

On what to do next for your next shot. I too thought .20cc would be better -- but that was still close to 4 hrs. I finally settled on .10cc for 1-2hr (and still good chub into 3 hrs).

So maybe just split the diff and try .15cc your next time...

I certainty agree with tigerfan53. There is a lot of good information in his message to you and others reading this forum.

With Trimix and Bimix it is a good idea to start at a lower dosage and work up very gradually.

With these medications, the results from increased dosages can be more exponential than linear once you near the correct dosage.

Judging from your first experience, although cause for concern, you are most likely a good candidate for injections. Just use caution and you should get along fine.

Thanks for all the very helpful answers. I think I'll try .15cc and see how it works. I still haven't "self injected", so a little nervous about that, but I think I'll do okay. I'll try again in a couple of days.

Clearly less is better. Did the doc give you instructions to consult on all changes? Mine will not do a "first time" injection after noon, as he wants a report at mid-afternoon, and meets you at the ER if needed.

.15 sounds better than .2 And consider that it may not work at all, but that may also depend on how freaked out you are the first time you do it yourself. This is very much a trail and error process. But I am amazed that he started you at .3.

Then again - there are different prescriptions. I was doing .10 to .15 with mixed results.

URO gave me a weaker prescription and now at .20 I'm good to go.

Sure glad this post popped up , I'm going in tomorrow for my first shot , I will keep a close eye on the Dr now.......I'm thinking starting with .05 ???

I think you are very wise to start at a low dose. I started at 0.1 ml and found that way too much. Settled in at 0.03 ml (3 units).

As others have stated, the "correct" amount varies for each guy dependent on several factors including the strength of your compound.

Because everyone's reactions are different, and there is no precise Trimix formula, no one here can say what your first dose should be. In theory, your doctor is prescribing the mix and dose based on your situation.

Also, talk to hime about the exact amounts and how that relates to the type of syringe you are given. Different syringes have different markings.

Last, your prescription label will probably have a dosage that has nothing to do with what you will use. That is because the law requires that there be instructions, and at the time they mix this up, no one knows. My uro puts .20 as the "prescribed" dose, but we both know .13 sent me to the ER. So don't do like some, and assume that what is on the label overrides the actual instructions you get from the uro. Unless you like the nurse at the ER.

Just wondering how they determine a starting dose after all of the overdosing stories.......sounds like a shot in the dark so to speak .

My formula is 30/1/10 (papa/phenol/Alpros). I think you should be fine starting at 5 units. That's about the lowest I have heard guys starting at. If you are too high it probably will not be high by very much and I would guess sudafed, or walking up and down the stairs will help (that's what I did when I tried 10 units). If you're too low, just increase slightly the next time, and keep increasing the amont you inject in subsequent injections, until you reach a satisfactory level.

By the way I use 50 unit syringes, they are easier to read than the 100 unit needles. Also, I use 31 gauge needles that are short (5/16 inch?)

Yep, my doc wanted to start at .5 ml, but after reading some posts I asked for .2 ml & we got a mild response. After getting the prescription, we tried .3 ml, very nice, took 2 hrs to go down with Ice packs. So it appears that the prescription is perhaps newer or was mixed stronger. Hope the readers of this will always start with a 0.1 ml dose or lower.

Just to be sure we all understand, trimix is a mix of three drugs that are mixed in various concentrations by various pharmacies. There are a couple of common concentrations but it's hard to describe them as standards. Your urologist knows the concentrations he has ordered and how many units represents how much of the drugs. Different doctors use different doses and it's hard to compare notes.

I started with one formula where the usual dose titration was to start at .2ml (20 units) and to work up to .5 (50) if needed. With that (rather watery) mix I would never say "20 units! Wow, that's a lot!" although there are occasional men for whom it may be too much. I found that I was tending toward the 50 unit end of that range and switched to a stronger mix to save a few bucks and reduce the volume I needed to inject. Based on the Alprostadil concentrations my new mix is 3.57 times as concentrated. 20 units of my new mix is 71.4 units of the old.

Adrenaline tends to interfere with erections and you may get less response the first few time you self-inject so take the dose increases slowly.

With self-injection I tend to remember my high-school chemistry. Do you remember "activation energy" in chemical reactions? That's what keeps the world from bursting into flames despite all the flammable materials and oxygen. There are two sets of forces in chemical reactions. There is a force that repels atoms so they push away from each other. It isn't very strong but it reaches out a long way. There is a stronger force that sticks atoms together so they react but it doesn't reach as far. The "activation energy" is how much work you have to do to push the atoms together to get past the weak repulsive forces so the stronger atomic forces can take over and cause a reaction. When you self-inject you will find that the penis and the needle tend to push apart fairly strongly. It takes some determination to get them together. But once the needle touches skin and your realize that 1) you can barely feel it and, 2) it doesn't hurt, then you have overcome the activation energy and the stronger "Yee-hah! Here we go!" forces take over. With experience you get better at dealing with the activation energy barrier but it is an intrinsic part of the nature of phallus and needle and will never altogether go away.

Chigh - No matter what dose your uro injects you with, be sure he doesn't send you out the door until your erection goes below penetration hardness ... and he's given you instructions on what to do for an erection that lasts over 2 hours.
