You hit the nail on the head. That's why Cathy Charles subtitled her original paper on the subject, "it takes two to tango." After millennia of doctors and patients interacting in what she calls "the paternalistic style," (Style #1), the information age has opened up new possibilities for interaction. Some doctors and patients will be resistant to the change. In #4, the doctor has to be willing to accept the legitimacy of his patient's values and preferences that may be entirely different from his own. But things are changing -- five years ago you might have had a hard time finding a doctor willing to work with you on Active Surveillance.

I have a team of doctors, each one I value. I tend to listen to my lead medical oncologist Myers. He certainly knows more than I do.

That was a great example, Tall Allen, as was the prior post a fine illustration of how many decision points there may be along the way.
I also see davidg's point that even a decision to defer making the decision oneself is a decision.
In your example I would choose #4.
