Focus on getting your health back, best you can, amidst the other distractions. When you are healthier, your problems will gain some perspective. Perhaps steer your wife toward some outside help. If she's not ready for that, think about (when you feel up to it) a support group (like Al Anon) for yourself.
But the most effective tool in my arsenal is prayer. It's really hard to focus on it now, but I know there is much good in your life. Spend some time in prayerful thanksgiving (good time of year for it) and let that goodness crowd out other thoughts for as long as you can, then ask your loving Father for some help. Then relax for a bit - at least several minutes of quiet - and see if anything seems clearer. Even if you hold no religious beliefs, the process of Thanksgiving for that which is good in your life, and quiet reflection afterwards, will help you cope.
