HW buddies, I found myoho's first post on another thread on behalf of her husband. He had a G9 1400 PSA diagnosis, and she also comments about his current situation.

myoho, posting here to give you your own thread. This way members can better give you direct responses.

Writing for my husband:

Diagnosed at 60th birthday: 7/3/12
Gleason: 9
PSA: 1400
Admitted with Renal Failure & metastasis

Lupron for 6 months; PSA back to 125
Casodex for 2 months; PSA back to 125

Currently enrolled in Clinical study combining Lupron, Zytiga and Xgeva, along with Demnosumab
PSA continuing to climb
Too early to say if effective

No chemo or radiation yet

Scans showed progression into hip joint and right femur, with some thickening over the past year

Curious if PSA is an indicator for Gleason 9? Saw a post earlier stating it may not be a good indicator?

Sorry for not responding earlier, although I am not very helpful to your question. There are many here that have worse stats that have had very favorable results from their treatments for many years. There are many negatives to this disease, but the positive is that it is slow growing and there are new treatments being introduced. This forum has been very helpful to learning of new treatments being developed, sharing experiences, and keeping a positive attitude. I hope you stay with us and keep us updated on your husband's progress.

I really don't understand the question . He has been classified a Gleason 9 by a biopsy I assume , tho I do not see that mentioned in post. He had a psa of 1400 and went done to 125, which is good news. It doesn't matter whether a psa score indicates a gleason 9. He has been diagnosed by a medical team as a gleason 9. Now , to me , there is a question of whether he ever had a biopsy. MyOhos, His gleason is a moot point at this juncture. He has an aggressive case of advanced cancer, and if his medicos, did not do a biopsy, tho that would be unorthodox, I personally see no need for it,. He has, as I said an aggressive case of advanced PCA. whoever Started the Hormon therapy{HT} did the right thing. I have a sense it was a local urologist or oncologist. They got it down to a psa of 125, which is good, but not great. He needs ,ASAP to be treated in a major medical center by a team of topknot urological oncologists who specialize in cases like his. Not just your local hospital, don't care How nice they are or how much faith you have in them. Get the ball rolling on this treatment . There are a lot of ammo left in the arsenal and its not too late, Im hoping. We have men here still alive quite a long time who started with twice his psa. forgive the lack of responses to your , post, Its the holidays and some are still making merry. If your husband has given up, make him read this post and start fighting for his life. His case is not good, but those with worse, are still alive, but he needs treatment above and beyond a few shots of HT, given by a urologist. If I am wrong about your medical team, I am sorry, and give us an idea of what there next step is for him, so we can give you our opinions on such steps. I again am assuming that is why you posted. If it was just to answer the specific question in your post, re. psa and gleason score, that is in my post as well. Good Luck and He can live for a lot more years with proper treatment.

Yes, PSA and PSA kinetics are still useful indicators. Sometimes it will climb, but at a diminished rate. They will also look for signs of radiological progression, as well as other biomarkers like alkaline phosphatase. While it is true that higher grade tumors - like GS9 - sometimes put out less PSA than lower grade tumors, they still put some out.

I think that is an incredible combination of medicines he is on, and wish him all the best with it.
