Ziggy,
I went thru a long battle with my insurance re qualification for Provenge - it took 2 appeals and an in person visit to the appeals board, but I prevailed. Started Provenge on 3/4/14.
The thing that won the day for me was the fact that, like you, I had a previously diagnosed metastatic lesion (to L2 vertebra). This was treated with spot radiation, and now does not show as metastatic. I found 2 journal published articles that stated 'once the patient has been diagnosed as metastatic, he retains the diagnosis even if the metastasis is no longer present'. This was the turning point for my argument, and it convinced the panel.
As long as you have a diagnosis of metastatic disease, and you can prove 'disease progression while on androgen deprivation therapy', you should be qualified for Provenge.
