I think you should discuss with his urologist ruling out other causes of high PSA, like prostatitis or BPH, before getting a biopsy. How big is his prostate? At minimum, his doctor can check his blood first to see if he has a high neutrophil-to-leukocyte ratio. Also, he can ask his doctor for 10 days of Cypro to see if that brings his PSA down. Then recheck his PSA after a month - I would use the new PHI test (which includes both PSA and % free PSA). You can arrange for it at the following link:
