Low T, high PSA

I'm 44 and healthy, last year I started noticing signs of low T, I had it checked and it was 343. My family Dr wouldn't put me on TRT, but my urologist agreed to ( I see a urologist for kidney stones.)
I've been on Androgel for a year and it's helped a lot. I don't know what my T level is, but the symptoms are gone. I went for a check up today and my PSA 3.5 and the urologist wants to do a biopsy. He said nothing felt enlarged, he says the PSA is borderline high. I looked up the biopsy procedure and it's less than pleasant to say the least. I looked at things that can cause elevated PSA, one of them being frequent sex. I have a beautiful wife with a high libido, we probably have sex 5 times a week. I really don't want a biopsy, but I also don't want cancer wither, any advice is appreciated.

It is advised to not have sex 48 hours prior to a blood draw for PSA purposes. Other than that brief time span, enjoy. Your PSA could be a bit elevated for reasons that are not PC. BPH, prostatitus, riding a bike prior to the blood draw and as you stated, maybe you had sex just prior to the blood draw. At your young, healthy age, I am betting on something other than PC.

A biopsy is nothing to fear but, instead a way to rule out PC and move on with your life.

Your  comment reminds me of a quote a very famous doctor who specializes in prostate cancer once said:

 

"Tell me what you want to hear and I'll refer you to someone who will recommend it."

 

 

 

So I'm curious...did your family doctor tell you why he would not put you on TRT?

Your PSA is borderline high. I would have it retested making sure you avoid ejaculation or riding a bike for 48 hours first. If it's still 3.5, you can have it retested in a year or have a biopsy rather than worrying about it. A DRE is not reliable unless the tumor is in the reachable part if the prostate. The biopsy is NOT painful if done by an experienced urologist !

There's way too much biopsying going on. It results in a lot of unnecessary suffering. Avoid sex and bike rides for 2 days before a re-test. If still elevated, investigate whether prostatitis may be to blame first. The TRT may be causing a rise in DHT levels that can be giving you some BPH - avodart or proscar can control that if that is the case. There's also a new blood test called PHI that is covered by insurance. Biopsies are good when you need one, but a lousy idea when done without due consideration.

Although my T was low my doctor and endocrinologist did not just prescribe replacement therapy, I had my testes scanned and needed two brain MRIs to determine WHY my T was low. The initial scan showed some "marking" on my pituitary gland, the thing which regulates T. Second scan showed none. (Note, all the diagnostics were FREE)

Now as far as having sex 5 times a week is concerned, I cant help you with that other than suggesting that you keep on doing it, as frequent sex safeguards against PCa. www.harvardprostateknowledge.org/does-frequent-ejaculation-help-ward-off-prostate-cancer
