Another possible reason for the "trickling" could be that he has strictures (scarring) from the surgery. That would be even more likely if he has had RT, but he probably hasn't had that yet.
Does he have a full flow first thing in the morning? The bladder becomes more or less dormant at night so it fills up. With 400 to 500 mls, he should have a full flow.
As Peter says, incontinence also results in bladder degeneration. I am at the point where I have a strong urge to pee when there is only 150 to 200 mls present. A normal bladder wont get the need to void until there is 400 to 500 Mls.
It is possible to train your bladder, but it takes a lot of effort and discomfort. You just have to keep hanging on until the last possible moment, but that doesn't really work if you are incontinent.
The only way to fully know the cause of a weak flow is by cystoscopy or a Urodynamic study. The cystoscopy is predominently to show strictures, but the full study can also identify weak sphincters, weak bladder etc
