It does seem a little concerning and it's good to know that he has someone in his life who cares for him. Although it is a rise, it is a small rise, one that should be watched carefully. He may be looking at some additional therapy in the future but it appears that it will be able to be managed for a long time. Best wishes to you and your husband.

Based on my vast knowledge and authority as a "Random Guys on the Internet" here are my thoughts:

Since his PSA remained detectable after surgery something was left behind in surgery.

Since it behaved itself for ten years whatever was left behind is fairly well behaved.

Based on the recent creeping-up of his PSA it is possible that the missed bits are finally considering making trouble.

This might be a good time to start asking around for a local radiation oncologist, just to have one in your Rolodex.

Thanks everyone for your thoughts. I can't tell you how much I appreciate your input. I learned so much about this topic 10 years ago when we were in the throes of it! I rarely asked questions or commented but was a "lurker" and a pray-er for many of the "old timers". I went back and looked at my records. He Was actually diagnosed in 2005. Following surgery, PSA did go down to<.10, <.04, .07, 0.11, .07, and then hovered as noted. Last 2 readings were 0.11 and 0.14.
Do these initial readings post-surgery change your thinking at all? also was on Casodex for a 16 week period immediately following surgery due to close proximity to urethra.

I was nearly 8 years between my original surgery and SRT. My PSA hit a low of .02 after surgery and stayed in that vicinity for several years -- then jumped to .1 and stayed there for a couple more years. But then it moved quickly to .2 and then .4 within a few months. Based on the guidelines at the time most doctors didn't consider SRT before PSA reached .2. But based on some of the recent information Tall Allen has posted and the greater accuracy of testing these days, it looks to me like now is the time for you and your husband to consult an RO.

Tumor was confined, both nerves spared, surgical margins were negative for tumor. Gleason 3+4. Lymph nodes were clear.
I guess we will wait and see what next month's PSA test looks like.
