im 41 and just got my results back

hello everyone,
im 41, my psa is 1.4 and free psa 0.3 and my free psa percentage is 21%

USS reviled my prostate gland is enlarged and is 35cc and mildly indents the floor of the bladder, small foci calcification are present within the gland.

urine test showed nothing all clear no infection.

i do go to the bathroom often at night.

could someone shed some light please, just want to know of i need to be concerned or not.

Yeah, you're going to the bathroom a lot. You should get that looked at.

Cancer? Seriously doubt it.

BPH, enlarged prostate could easily account for what you've got. The docs have plenty of treatments for that.

Is your doctor concerned? What are the metrics that are making you concerned. Also, is it pca that is your concern or your prostate health generally? The larger prostate would account for a psa of 1.4. The prostate "denting" bladder would account for frequent urination. I have no prostate and urinate frequent ly at night. Free Psa of 21% is mildly concerning as it is not >25%. Close enough for the most part.imo.Have regular urological visits with a uro you are comfortable with.

I am not aware of any evidence that percent free psa is relevant where total psa is below 2.0.

I was in a similar position to Gorlia when I was 41. I had a total psa of 1.4, nocturia, and my father had prostate cancer. My DRE was normal.

I was concerned but I concluded there was nothing to do about it other than to monitor my psa regularly. I have done that for a decade. The highest it got was 1.75 and the most recent was 1.5. If it goes up, I will get  a biopsy. Until then just continue to watch it...

A 35 cc prostate can generate up to a psa reading of 5.2 due to benign psa. The psa density ratio of 1.5 is the cut off that most knowledgable doctors use to determine if psa is benign due to BPH. A concern in your case would be a psa reading above the 1.5 ratio or above 5.2 psa.
If frequent urination is bothering you, than Adovart or Proscar to shrink the prostate would be appropriate and a new psa base line established after taking for a few months.
You have all the classic symptoms of BPH and if it walks and quacks like a duck it's a duck.

Gorilla,

The BPh that John is talking about stands for "Benign Prostate Hypertrophy" and it means your prostate has grown a bit too big but is otherwise mostly OK. A big prostate can make it hard to pee and can make it hard to empty your bladder completely when you do pee. This is probably what keeps waking you up at night -- you go to pee but you stop when your bladder is half full and it isn't long before it fills up again.

There are several medicines you doctor can give you to a) make it easier to empty your bladder, and b) shrink your prostate. They work pretty well and aren't a big deal. The drugs to shrink your prostate can, occasionally, put a dent in your sex life so be sure to talk to your doctor about that if you notice a problem, but most guys do fine.
