Just as a quick guess, the hazard or odds of something "causing" or being associated with whatever the bad thing is you are looking at, compare to some control.
So, the group with lowest amount of X being called 1 has an HR of PC of 0(or maybe 1), and the highest group(lets say 4 groups) with the highest average amount of X has an HR of 3.5 or 3.5 times as likely to get PC?
Then the adjusted HR would probably be adjusted for various other things associated with PC, like age or who knows what else.
That is just a quick guess until someone who knows checks in here.
