I'm a G9 with pelvic lymph node involvement diagnosed in July 2013. I have been on ADT since then, and did IMRT in early 2014. My PSA began rising from <.05 late last year with a one-month doubling time, indicating that I had become castrate resistant and that the cancer was growing again rapidly. On the advice of several MOs I entered a Phase II trial of ipilimumab, a checkpoint inhibitor, last month. Ipi is one of the immunotherapies that's been getting a lot of attention lately, so I thought folks in this forum might be interested in following my results. So far, so good. My PSA is down from 5.29 when I began the trial on June 10 to 4.5 today. Thus far, no real side effects. I'll update my results as I go through the course of treatment, which involves four initial infusions three weeks apart and quarterly infusions thereafter for as long as the drug is working.

That is of interest. It looks like ipilimumab has not been tried often in PCa but has been successful in malignant melanoma. Since it is phase II is there a placebo control?

Good luck with the ipi - great stuff! I think the combination of immunotherapies has a lot of potential, but also potential toxicity - so clinical trials like yours will give us lots of valuable info.

Are you allowed to try other hormonals (Zytiga, Xtandi) while you're on this clinical trial?

I appreciate the good wishes. This particular trial involves my continuing with Lupron (I'm also on Xgeva, which is OK), but not adding any of the newer AR pathway drugs like Zytiga or Xtandi. Also, no vaccine - though eventually the combo immunotherapies look like the best bet. They want to see what happens with ipi alone added to first stage ADT. They're going to be looking in great detail at what happens to my immune system - lots of blood draws each visit. There's no placebo control, which I'm happy about - at least so far!
